SPECIAL POWER OF ATTORNEY

KNOW ALL MEN BY THESE PRESENTS:

That, I, _______, Filipino, of legal age, with residence and postal address at ___________________________________________________________________________________, do NAME, APPOINT AND CONSTITUTE, as I have NAMED, APPOINTED, AND CONSTITUTED, my legal Spouse/ Child, ________________________________________________ Filipino, of legal age with residence and postal address at ____________________________________________________________________________________ to be my true and lawful Attorney-in-Fact singly for me and in my name, place and stead, to act as the administrator of my Personal Equity and Retirement Account (PERA) to be opened with BPI Asset Management and Trust Corporation (“BPI AMTC”). As administrator of my PERA, my Attorney-in-Fact shall perform the following acts and things, namely:

1) To apply for/open and operate my PERA (the “Account”) with BPI AMTC under such terms and conditions acceptable to the Trust Corp., which Account is in my name;

2) To deposit or invest monies and/or funds in any amount and to withdraw the same by way of credit to my nominated BPI settlement account, or by check; and to operate, deposit to or withdraw from my Account, as well as to sign debit and credit memos/instructions and all other instruments and documents;

3) To sign, execute and deliver any and all documentary requirements that may be required by BPI AMTC that is necessary for the opening, operation and withdrawal of any amount in the Account;

4) To perform any or all acts necessary for the continuance and/or termination of my Account with BPI AMTC.

HEREBY GIVING AND GRANTING unto my said ATTORNEY-IN-FACT full power and authority to do and perform every act and thing whatsoever requisite or necessary to be done in or about the premises as fully to all intents and purposes as I might or could lawfully do if personally present, and HEREBY RATIFYING AND CONFIRMING all that my said ATTORNEY-IN-FACT, shall lawfully do or cause to be done by virtue of these presents.
