Make sure you give your counselor plenty of time, at least two weeks, to write your letter of recommendation. We recommend that you complete your questionnaire the summer before your senior year. Keep in mind it is our job to show the college how you influenced BCHS and our surrounding community. They want to know how you will be a positive addition to their college campus. It is our job to make you stand out to college admissions, so give us examples and details!

INSTRUCTIONS FOR STUDENTS:

1) Complete the Counselor Questionnaire in a timely manner be sure you are aware of college deadlines. They do not take LATE applicants. Return to your counselor by email or in person. Please TYPE your answers in full sentences and number your responses according to the question.

STUDENT SECTION:

**Note: College admission officers are more impressed by QUALITY rather than QUANTITY in activities!

1. Your full name, phone number, and email address.

2. What is your current career goal? What is your intended major?

3. List the colleges to which you plan to apply. Explain why you decided on this group of colleges. What do they all have in common? (Think about your career goal and the mission of the school). What is the most important consideration for your choice of college?

4. Please share why you took the courses you did in high school. Why did you to take AP, Honors or dual? If you took all or a majority of general education classes explain why.

5. Was there a class you struggled in due to circumstances out of your control (multiple teachers? A substitute for an extended period) was there a conflict in your schedule that did not allow you to take all the AP/Dual classes you wanted to.

6. What makes your family unusual, different, and/or special? Any family hardships or challenges? Do you live with both parents? What is each parent’s highest level of education?

7. What are your personal strengths? Give examples that document them! What makes you different from all other applicants?

8. List your leadership roles/position (on or off campus): What have you done to demonstrate your leadership? Describe how this role has impacted your life. Has it impacted your career goals?

9. Explain how your experiences in high school have influenced your choice of college and how you will be active on a college campus.

10. How have you grown or changed during your high school years? How do you want to grow in college?

11. Are there any issues that you would want your counselor to explain in the letter of recommendation for you? If so, describe the issue.
