The vision is for an integrated model of community–based health and social care to provide seamless services to the people of Conwy & Denbighshire which will be sustainable after the transformation is completed. The programme is utilising funding from Welsh Government Grant to establish 4 x Integrated Health and Social Care Localities and build on the work to date of the existing 9 x Community Resource Teams to refine and develop new ways of working that support the integration of service delivery.To achieve this vision a short-term multi-disciplinary Programme Team is to be formed which will comprise:-District NurseSocial WorkerChildren ServicesPharmacy TechnicianTherapy LeadVoluntary Sector RepresentativeGP/Primary Care LeadFinance TechnicianBusiness AnalystThe team will be supported by Programme, Project and Business Support in delivering the Integration and Transformation Strategy. The purpose of the Project Manager is to:-Ensure the success of the Transformation & Integration Programme in accordance with agreed partnership approach, budgets, timescales, and achievement of the desired outcomes. Ensure effective partnership working and develop robust strategic and operational contacts in order to present effective integrated care. To manage change constructively and successfully, ensuring that services work together to have ownership to improve the health and wellbeing of each community.The post holder will be required to plan, support, facilitate and monitor the progress of projects, products, service improvements or change management initiatives, providing management support, training and expert guidance to partnership colleagues. To ensure that individual project objectives are met and are delivered within the required standards, timetable and budget.To support the programme team in developing systems to capture and report service improvement and benefit realisation. To contribute to and support the delivery of the Transformation and Integration Programme Objectives. To work closely with other stakeholders and delivery partners as well as members of the public. These will include but not be limited to departments in the Conwy and Denbighshire Local Authorities, the Health Board, other public sector organisations, independent sector providers and developers; voluntary sector organisations, local communities and Elected Members.To plan for the sustainable delivery of new ways of working beyond the lifetime of the project and programme period.Duties and Responsibilities - Job Specific1STRATEGIC CONTEXTTo understand and promote the strategic direction for Community Services Transformation Programme within the central Area. This will include:-Keeping up to date with national guidance and corporate strategy - e.g. Well-being of Future Generations (Wales) Act 2015, Health & Wellbeing Act (Wales) 2016, Parliamentary Review, and Healthier Wales - and interpret the impact on local projects. Ensuring that relevant projects and work-streams are established to deliver strategic objectives. This will involve planning a broad range of complex activities which involve uncertainty.2PROJECT MANAGEMENT2To be responsible for all aspects of delegated project management for projects and work packages. This will entail: Managing a specific project, product and or work package, taking appropriate action e.g. to manage issues, risks and deviations to plan to ensure that projects and products are delivered to schedule. Planning, monitoring and controlling projects or work-streams throughout the project lifecycle to deliver benefits using structured methodology. Taking decisions (within delegated authority) on a range of complex project issues where there may be more than one course of action. Advising more senior staff on the optimum solution to resolve project issues for more complex projects Ensuring project, product and work package delivery by providing management, direction and support to the Programme Manager and senior management in Central Area, who are engaged in the planning and delivery of projectsCo-ordinating the requests for change required within the post-holder’s portfolio of projects.Co-ordinating project teams/work-stream teams to ensure that all personnel involved with project activities understand their roles and carry them out effectively, taking corrective action where appropriate. Completing or ensuring that the project team(s) accurately completes and maintains all Project Management processes/tools including business cases, specifications, project plans, benefits analysis, and risk and issue logs. Production of or contribution to a variety of high quality reports required for the successful management of projects e.g. Highlight, Exception and Check-point Reports.Ensuring that all areas of work are carried out in an economic and efficient way thereby contributing to the control of limited resources.Leading on a project or work-streams for performance improvement across sites, imparting sometimes unwelcome news to stakeholders e.g. changes to timescales, or additional work required to tight deadlines. 3COMMUNICATIONProvide and receive complex, information relating to project or work-stream, e.g. changes to agreed timescales and local impacts of these. Use interpersonal skills to advise Project Boards, Senior Managers, Executive Teams and Elected Members and other staff of the importance of project or work-stream, using persuasive and motivational skills to overcome any barriers to change or issues. Deliver routine/ complex presentations to large and small groups on project / work-streams, including knowledge in relation to other operational/strategic initiatives. Ensure that effective communications for the project are maintained within the department and project teams, updating staff on any relevant project issues. Support the Programme Manager in delivering high quality reports/ plansCommunication with external bodies, being aware of sensitivities and political considerations. Challenge project staff (at all levels) to deliver their committed actions, undertaking negotiations where required 4BUSINESS ANALYSIS/ SERVICE IMPROVEMENT/ CHANGE MANAGEMENT Identify opportunities for the introduction of new service models to transform working practices, using knowledge to inform the scope and direction of projects/ work-streams that they manage.Manage/undertake diagnostic work, as required e.g. to identify requirements of a solution or changes required to support that solution. Lead on specifying service requirements involving key stakeholders and ensuring that comprehensive mapping of services is undertaken to understand ‘business needs as is ’ and any evolutionary requirements. Manage and deploy suitable techniques to bring about and support service/organisational change, ensuring that the case for change is robust and that the suggested way forward is owned.
