Physician Referrals

Straub Physicians:

? Please enter your physical therapy orders into EPIC with all required fields completed.

? We will contact your patient directly and schedule an evaluation within 3 ? 5 business days.

? If you require a stat order for your patient, please call the Straub Physical Therapy location of your choice to inquire about availability.

Outside Network Physicians:

? Please use our printed rehabilitation prescription form and complete all required fields.

? Fax your prescription form to the Straub Physical Therapy location of your choice.

? We will contact you patient directly and schedule an evaluation within 3 ? 5 business days.

Physical therapy (PT) is a structured rehabilitative program designed to meet individuals' specific needs in restoring physical function.

About Straub PT

We specialize in the rehabilitation of orthopedic injuries, post operative orthopedic rehabilitation, balance impairments, vestibular rehabilitation, gait training and functional losses that are a result of physical or neurological insult.

Utilizing therapeutic exercise, patient education, manual therapy and modalities, we work to restore a person's full functional potential according to their specific needs.

Each clinic location is equipped with modalities, mechanical traction (except on Lana`i), private treatment rooms and a small gym.

Each patient is given quality 1:1 care with a licensed Physical Therapist for the entirety of each visit.
