Privileges to Include: (please list and be specific ? attach additional pages as necessary) PHYSICAL THERAPY SERVICES TO BE PROVIDED
1. Schedule the provision of services in consultation with patients and referring physicians.
2. Provide appropriate physical therapy diagnosis, prognosis, and intervention services to patients in accordance with physicians' orders, all applicable laws and regulations, and applicable Kaleida Health policies and procedures.
3. Properly document all physical therapy services provided to patients under this Agreement in a timely manner.
4. Mentor, train, supervise and assist physical therapy personnel at Kaleida Health rehabilitation sites, as requested by Kaleida Health, in developing and using treatment strategies, developing sound working relationships with physicians and patients, and other matters relating to physical therapy diagnosis, prognosis and intervention.
5. Participate in departmental quality assurance initiatives pertaining to peer chart review in accordance with the departmental policy titled: Documentation/Chart Reviews (QA).
