Creating Follow Up Appointments for Nurses

What is it?

Cerner PowerChart EDUCATION

? An area to add scheduled appointments or other items the patient needs to follow up on post discharge. ? The follow up area is usually completed by the provider but can be completed by Case Management or Nursing. ? Follow up is required prior to printing the Patient Discharge Instructions.

Where is Follow Up Located?

1. Go to `Nurse View' on the dark menu on the left.

2. Select the `Discharge-Nursing' tab from the top.

3. Select `Follow Up' from the light menu.

Completing Follow Up: Quick Picks

Quick Picks ? If the patient's Primary Care Provider (PCP) is known it will show up here. 1. Click on Quick Pick you would like to add. 2. The selected item will move down to the Added Follow Ups Area.

Modifying Added Follow Up 1. When you select the PCP, no follow up information will populate. It is necessary to click on `Modify' in the Actions

column to add those details.

2. Select the Time Frame. 3. If the appointment is scheduled select the date. 4. Enter necessary information if not already included. 5. Enter a comment either free text or choose one of the predefined comments. 6. If you would like to save the template for future use select the check box next to "Save as Template". 7. Select `Save'.

Saved Templates ? you can find you saved templates under Add Follow Up. Click on `Saved Templates'.

Completing Follow Up Using the Window When entering follow up you have the option to utilize the follow up window: ? Click on the + to the far right of the follow-up screen.

? Enter follow up information 1. Search for and select the intended provider in the Who section. 2. The PCP listed in STAR registration will default into the follow up window. 3. Enter a timeframe in the When section if a follow-up appointment is needed or already scheduled. 4. Complete the Where section. If a provider has multiple locations, select the intended address. 5. Add any comments as necessary by double clicking on the predefined comment. 6. Multiple providers can be added/selected for follow-up appointments. To remove a provider, click the in the Selected Follow up section. 7. Select `Sign Pt. Education' to add the follow up appointment which will be included on the Patient Discharge Instructions.
