AmeriBen is pleased to announce that we will be the new Third Party Administrator for Georgia Baptist Convention effective July 1, 2015. AmeriBen will now be responsible for the administration of your medical benefit plans. This includes claims payment, benefit inquiries, pre-certification, and various other functions as defined by Georgia Baptist Convention.

What is a Third Party Administrator? A Third Party Administrator (TPA) is the party responsible for the processing of claims and application of benefits according to the Plan Document set forth by Georgia Baptist Convention.

How does this change affect me? As a member you will be receiving a new identification card(s). This card should arrive by July 1, 2015. Once you have received this card please destroy any current ID card(s) and present your new AmeriBen ID card to doctors for services rendered after July 1, 2015. You will also have a new toll-free phone number to call. The AmeriBen Customer Care Center is available to answer your questions about plan benefits or to provide information concerning the status of your claims Monday through Friday, 7:00am to 6:00pm Mountain Time or 8:00 AM to 5:00 PM Pacific Time. The Georgia Baptist Convention toll free phone number is 1-877-955-1577. You can also log on to to submit an inquiry or view your claims history.

Where should my provider(s) send my claims? For any claims, with a date of service after July 1, 2015, your provider should submit claims to: Anthem Blue Cross Blue Shield PO Box 105187 Atlanta, GA 30348-5187

Why does AmeriBen require my dependent's Social Security Number? Federal law requires medical plans to report Social Security Numbers for all covered members to Medicare. If you are enrolled in the Georgia Baptist Convention medical plan, you are required provide the Social Security Number for each of your covered dependents. The Medicare, Medicaid and SCHIP Extension Act of 2007 mandated this reporting requirement. The law is described in detail on the CMS Website: cms.mandatoryinsrep/
