Protocol for Accessing the Port-a-Cath
General Information – An implantable venous access system. It is surgically inserted beneath the subcutaneous tissue on the chest wall. The port is made of titanium. Within its chamber is a reservoir. A polyurethane catheter exits from the port and is tunneled through the skin. The tip of the catheter is inserted into the subclavian vein and advanced to rest in the superior vena cava.
Usage – Administration of Intravenous medications, IV fluids, Blood and blood products. Blood sampling can be obtained with the device.
Precautions – Use only non-coring needles, use a 10cc or larger syringe when accessing the port.
Accessing the Port
Gather equipment
Central line dressing kit
Non-coring needle
Extension set/catheter hub
20cc syringe with needleless adaptor
10cc syringes with needleless adaptor (2)
Flush solution (Adult - NS 20cc/Heparin 100u/ml 6cc)
(Pediatric – NS 10-20cc/Heparin 100u/ml 3-4cc)
Explain procedure to patient
Wash hands
Open central line kit
Place equipment within sterile field
Don sterile gloves and face mask
Clean skin with swabs in a circle motion from port outward
Draw up saline and heparin into syringes
Attach non-coring needle to extension tubing with catheter hub
Attach one of the 10cc syringes to the extension tubing and flush the line
then clamp it closed.
Anchor the port-a-cath with 2 fingers
Grasp the non-coring needle by its wings and insert it into the skin, then into the port-a-cath hub and continue to insert until you feel the base of the port.
Unclamp the line and pull back on the syringe to verify blood return. Flush the line with saline. Clamp the line. Attach an IV line for an infusion of fluids or flush the line with hep lock solution if not being used.
