Differentiation of activity is to be provided throughout the lesson plan to ensure that children’s individual learning needs are met.

1. Learning Objectives

List desired student outcomes here by starting with, “The students will be able to (SWBAT)…” Remember to use Bloom’s Taxonomy to identify exactly what the students will do to show what they learned.

2. Assessment

(a) Learning outcomes of previous lesson related to this topic:

List information students need to know from a lesson that’s already been taught.

(b) Focus of assessment in this lesson (related to objectives)

Please be very specific and list the skills and/or concepts that will be the focus of the assessment. Be sure to look at what you’ve listed as the objectives to help you here. Be sure that the objectives and this section directly correspond with each other.

(c) Method of assessment used in this lesson (example attached)

Please state exactly how you will find out what students have learned (from the objectives you’ve listed above) during this lesson.

(d) Differentiation (of expected outcomes)

How will you be sure that different types of learners have access to learning what you’ve planned to teach? In this section, start by identifying what you will include in your lesson that represents your content for students who are (1) visual, (2) auditory and (3) tactile learners. If you know of specific disabilities that are represented in your classroom, be sure to identify ways that you will accommodate such differences in your lesson as well.

3. New York State Learning Standards

Use as a resource to align your lesson plan’s desired student outcomes to the NYS Learning Standards. Be sure to identify the standard(s), the key idea(s) and the performance indicator(s) and write out each completely.

4. Materials

Make a detailed list of all materials that you and your students need for your lesson. Please be sure to include how many of each item you’ll need. Attach copies of all student handouts you create and/or use and be sure to note what you’ve attached here. Please also attach copies of the covers of texts, books, and other materials that you've chosen as resources for planning this lesson.

5. Lesson Process

(a) Introduction (approximate time) /Links to prior knowledge:

Write to tell how you will introduce your SOCIAL SKILL to your students in a motivational way (learning hook). Write to tell how you will elicit students’ prior knowledge on the subject matter (Remember, choose 1 social skill: listening, turn taking, asking for help, asking questions, giving praise, for example).

(b) Learning procedures relating to objectives (approximate times):

Write a detailed sequence of learning experiences as a timeline including all learning experiences that your students will complete (The experiences should require the use of the SOCIAL SKILL that you’ve introduced above). Choose an activity that requires students to work in cooperative groups (pairs are easiest to start with) that will require them to practice using the social skill you’ve mentioned above.

Be sure to explain a specific way that you will group students and be sure to identify how you’ve selected your student groups.

Be sure to include reading a children’s literature book that in some way pertains to the SOCIAL SKILL that you’re teaching.

Use care when considering time allotments for students’ tasks and learning experiences. Be sure to include a detailed description of your plans when outlining this sequence of events.

Be sure to list all directions, planned questions and targeted answers that will guide students through your lesson.

Write to tell how you (or students) will review the important concepts that are part of the SOCIAL SKILL that you’ve taught to begin to close the lesson.

(c) Closure/Conclusion (approximate time)/Checking for understanding:

How will you check for students’ understanding at the end of the lesson? Be sure to provide a way for students to talk whole group to debrief their use or non-use of the SOCIAL SKILL that you’ve discussed in this lesson plan. You may also want to provide students with a simple survey or questionnaire to find out how their use and their group member’s use of the SOCIAL SKILL went for them.

6. “If time”/ extensions

If time: Provide an additional activity that corresponds with the desired student outcomes that can be completed if you have extra time.

Extensions: Write to explain what experiences you will encourage students to complete following the lesson while they are not with you. If you created a take-home activity, please attach a copy of it. Please be sure to attach a copy of anything you’ve planned on sending home with students.

7. References (sources used in creating lesson plan)

Write a list of all references that you used to help you write this lesson.
