Any services specific to Coalfields?  - There aren't any specific services for the coalfields area as Telehealth and Telecare services area available across the whole of Sunderland (Telehealth team also work in Gateshead and South Tyneside).

 

A brief outline of what is on offer and which type of patient is best served by Telehealth/Telecare?   Telehealth services use technology to help patients live more independently at home.  This includes the use of health-monitoring devices that measure blood pressure, blood oxygen levels or weight.

Telehealth services are especially helpful for people with long-term conditions who would normally be monitored in GP surgeries, outpatient clinics or visited by community matrons or other health professionals.  They can provide peace of mind that you are safe in your own home and that your health is stable without you having to make regular visits to your doctor’s surgery. They can also help you live independently in your own home for longer, so you can avoid a hospital stay or put off moving into a residential care home.  

Many conditions can be suitable for monitoring and could include: Hypertension, COPD, Heart Failure, Diabetes, Smoking, Weight Management, Asthma, Parkinson's and many more. 

 

How patients can access services?  - Patients can only access the services through their health care provider. This could be their GP, Practice Nurse, Community matron, Specialist nurse.  We are also working with Public Health around Smoking Cessation so they could be referred to the service via that method.  We recommend patients enquire with their health care professional to find out more information.
