Enhanced Surgical Site Infections Prevention Bundle element (identify one per sheet): Bathing or showering with chlorhexidine gluconate (CHG) soap for at least three days before surgery
When we joined Project JOINTS, our Surgery Section Committee was already investigating preop bathing with CHG. After researching different products and discussing patient compliance, we decided to try SAGE wipes. We began by educating the nursing staffs in both the surgeons clinic setting and our own Perioperative department.
We (the hospital) provide (at no cost to the patient) a package of SAGE wipes to the patient for use at home the night before surgery.. The morning of surgery they are given another package shortly after being admitted for use immediately pre-op. At this time, on the recommendation of our Infection Prevention and Surgery Section Committees, we are beginning with the CDC and AORN recommendations of bathing the night before surgery and the morning of surgery. We will evaluate patient compliance before recommending changes in number of days patients wash pre-operatively
One package of wipes is given to the patient in the surgeons office when they are seen pre-op. The patient education is also initiated at that time. We created a patient information and “instruction for use” document that is included in our pre-operative instruction packet. The second contact with the patient is made by our Perioperative Service Pre-op Nurse. She will re-enforce the teaching that has begun and document in the patient record.
Beginning on July 18, 2011, Any patient (Class 1 below the neck) was given the wipes. Patients already scheduled were not.
Because of our small size, it was decided to start with all patients , not just Total Joint.
Even though our instruction sheet was thought to be very inclusive , the patients brought up questions we hadn’t thought of: 1. Can the wipes be microwaved because they are cold? 2. Can and should they be used on genitals? We are all here to learn from the patients.
