Any Native American Tribal Member Youths are Welcome to Participate! Boy and Girl Divisions- Age Groups: 5 to 12 years : 13 to 17 years
I, the undersigned, parent/guardian of the Contestant in the Salmon Festival Noo-rey-o-won-ee (Beautiful girl inside and out)/Keet-ko (Strong/Able boy) Contest, hereby accept the responsibility for all monies collected by said contestant as a result of ticket sales. I further understand that it is my responsibility to see that the proceeds from the ticket sales, along with any unsold tickets, are to be delivered to the appropriate person(s) in the KLAMATH TRIBAL OFFICE no later than the specified closing date of ticket sales for the contest (WEDNESDAY, AUGUST 17, 2011 at 12:00 noon). Tickets will only be issued to the contestant and/or parent/guardian of the contestant. Parent/Guardian is liable for payment for any tickets not sold and not returned to the Yurok Tribe. Please attach Tribal Enrollment Verification for the contestant. Ticket Price: $1.00 Per Ticket All contestants must fill out an entry form. The division entered will be determined at this time. A parent or guardian must give written permission to enter and sign a responsibility statement for all ticket monies collected. Tickets will then be issued, no more than 200 at a time. All money for tickets sold- complete with names, addresses, and/or phone numbers, must be turned in prior to issuance of the next series of tickets. ALL TICKET SALES END ON WEDNESDAY, AUGUST 17, 2011. MONIES AND REMAINING TICKETS MUST BE TURNED INTO THE KLAMATH TRIBAL OFFICE NO LATER THAN 12:00 NOON. TICKETS MUST ALREADY HAVE THE NAMES FILLED IN PRIOR TO THE DEADLINE. NO EXCEPTIONS. Contestants Must: Advertise raffle and contestant by poster; Not be married, have children, or living with a cohabitant; Sell a minimum of $250 in order to place/receive a percentage of the proceeds. Contestants/Parents are required to participate in the construction and preparation of the Salmon Festival Float for the Yurok Tribe Klamath Salmon Festival Parade on Saturday, August 20, 2011.
