This Directory contains a list of quality health care providers that are participating in the CMS Plan in your area, including primary care physicians, hospitals, pharmacies and other health care professionals. If you would like to know your provider's hospital affiliation(s), please call their office to get the most up-to-date information. CMS Plan, together with the two Integrated Care Systems (ICS): Ped-i-Care and Community Care Plan (CCP) is continually expanding our network of health care providers to improve access to quality health care services for our members.
