WELCOME TO CLEAR HEALTH ALLIANCE!

First, you need to pick a doctor. The doctor will send you to a specialist if needed. There are a lot of doctors to pick from. You should also pick a doctor for each member of your family. If you are living with HIV or AIDS, you may want pick one of the doctors with this symbol () from the PCP section of this directory. Many of our doctors' offices have been named a Patient Centered Medical Home (PCMH). This means the office will help you with doctor appointments, community services and other care for your family. The list will tell you the doctor offices that are a PCMH. Next, make an appointment with your doctor. This will give him or her a chance to get to know your health. Remember, you do not want to wait until you are sick to pick a doctor. All of your health care is provided at your doctor's office or through written referrals from your doctor. In accordance with Section 1932(b)(3) of the Social Security Act, your doctor and other health care professionals may advise you about your health status, medical care and treatment regardless of whether the benefits for such advised care or treatment concerning your health status are covered by Clear Health Alliance, however please refer to your Member Handbook for the services that are covered by Clear Health Alliance. In an emergency call 9-1-1 or go to the nearest Hospital Emergency Room. Sometimes you may have a medical problem that is not really serious and your doctor's office is closed. For those times you can use an Urgent Care service. Check this book or call the Clear Health Alliance Hotline at 1-877-577-9043 for a list of offices. At Clear Health Alliance, we want to get to know you so that we may give you the best care.

The doctors in this list do not work for Clear Health Alliance. Some doctors may not perform services based on religious or moral beliefs. Your book must be looked at to find out exact terms, coverage, limitations and exclusions. Note: The doctors and hospitals in this book are updated as of the day the book is printed. Sometimes new doctors are added to our plan or others may leave our plan. We advise that you call us at 1-877-577-9043 or visit our website at to make sure that the doctor is in the Plan.

REFERRALS

Your Primary Care Doctor is the doctor you will see first. If needed, he/she will send you to a specialist. Your doctors will work together for a treatment plan that will help you.

SELF-REFERRALS

You can go to the following specialty services without a form from your doctor: Chiropractic Care You can pick any doctor from this section for up to 10 visits within a 12- month period for members over 21 years old. Dermatology Care You can pick any doctor from this section for up to 5 visits within a 12-month period. Podiatry You can pick any doctor from this section for an evaluation, up to 4 visits. Gynecology You can pick any doctor from this section for the annual well-woman exam. Ask your doctor for a list of GYN's. Any more visits require a referral.
