What is Fit for Life?

Fit for Life is a benefit that Lilly offers to start your journey toward overall wellbeing through convenient, easy-to-access tools and resources. If you are already on your journey, Fit for Life offers challenges and support to motivate you along the way.

> Find information, motivation and support to follow through with healthy changes.

The program is confidential, voluntary and offered at no additional cost to you.

Who can participate?

Fit for Life is available to employees and non-Medicare eligible retirees enrolled in the Lilly Health Plan, their spouses/ domestic partners and eligible dependents ages 18-25.

Can I participate if I am not on the health plan?

Non-Lilly Health Plan employees, their spouses/domestic partners and eligible dependents ages 18-25 can access the Well-Being Assessment, create a well-being plan and engage in Fit for Life resources such as the Prime Fitness Network.

Why should I participate?

Your well-being is important and everchanging. And sometimes you need a little encouragement to make it a priority. Fit for Life helps you to understand the true status of your health so you can be better prepared to address any health risks that may affect the quality of your life. With the right tools and personalized support, you can get help to take the little steps that keep you moving along the path to better health and a better you.

What does Fit for Life provide?

Fit for Life offers a variety of tools and support, including:

HEALTH SCREENINGS During your screening, which will be conducted privately, a health professional will collect measurements, including height, weight, blood pressure and waist circumference. A sample of your blood will be taken to determine your cholesterol and glucose levels, triglycerides and other factors that can lead to lifestyle-related health complications.

? Employees will receive an invitation from Employee Health Services (EHS) to participate in an annual preventive screening.

? Approximately two weeks after your screening, you may log on to fitforlife.lilly. com to get a personalized report highlighting your results and what they mean.

? You and your covered dependents may also choose to go to your personal physician if you prefer. Download a physician form at fitforlife..

WELL-BEING ASSESSMENT (WBA) The WBA is a confidential scientific survey you complete online. You'll respond to simple questions and statements. Then, you'll get:

? A snapshot of your overall well-being and the five elements that make it up (purpose, social, financial, community and physical)

? Personalized insights for reaching higher well-being

ONLINE WELL-BEING PLAN ? The Well-Being Plan is a personalized summary of your overall well-being. ? It offers actionable steps, personalized recommendations and focus areas to keep you motivated and on track. ? Additionally, you can create fitness, nutrition, and weight and stress management plans to promote healthy behaviors and help you reach your goals.

WELL-BEING CONNECT? MOBILE ? A convenient mobile app that can help you track your health and well-being progress wherever you are ? Stay connected to your personal health trackers, social network, messaging center reminder alerts and more.

HEALTH COACHING ? A series of sessions with health coaches to guide you to better well-being ? Personalized support to help lower your risks, manage a condition and change behaviors ? A health coach can be reached at 1-866-336-0498. Face to face health coaching is available for employees in the Indianapolis, IN area.

PRIME FITNESS NETWORK ? Access to more than 9,000 fitness centers nationwide through the Prime Fitness Network ? You are not limited to choose just one center; you can use your Prime Fitness membership to join as many fitness centers as you like, as often as you like as long as they are in the network.

RESOURCES FOR QUITTING TOBACCO ? Access to QuitNet?, the world's largest online community of individuals who have quit or are quitting tobacco ? Phone coaching sessions with an experienced counselor ? Email tips offering motivation and encouragement ? Free nicotine replacement therapy products

Rewards: Incentives to live a healthier life

Lilly employees, spouses and dependents age 18-25 have two ways to win prizes ? just for doing a few simple, healthy things.

Spouses and dependents enrolled in the Lilly health plan qualify for incentives as well.

DEBIT CARD DRAWING

Individuals who participate in quarterly well-being challenges will earn well-being points and will be entered into a drawing for a chance to win a debit card for each challenge.

One of every 100 challenge participants will win a $250 debit card.

GRAND PRIZE DRAWING Waived Health Premiums for 2017! If you are enrolled in the health plan, you could be one of 15 employees who win waived health premiums for 2017, just for taking part in Fit for Life. The top 10 point earners will win waived health premiums for 2017, and each time you or your eligible family members earn 2,500 points you'll be entered in a drawing for a chance to win one of the remaining five grand prizes. Earn points by:

1. Completing a health screening

2. Completing the Well-Being Assessment

3. Earn 2,500 points (each time you reach an additional 2,500 points it earns you another entry into the drawing)

How do I get started? 1 Participate in a health screening.

Here are some things to keep in mind when preparing for your screening: ? For valid results, don't eat or drink anything but water during the nine hours before your screening time.* ? Continue taking medication as prescribed the day of the screening.** ? Drink plenty of water to make it easier to give a blood sample. ? Arrive 5 minutes early to register. ? Allow about 15 minutes to complete the process.

2 Complete your Well-Being Assessment. The survey is a confidential questionnaire that evaluates your well-being by gathering information about your overall health. Please respond as candidly as possible for the most accurate results. To complete your survey:

3 Receive your personalized Well-Being Plan. After you complete your Well-Being Assessment, you'll receive a personalized plan. Based on your responses and health risks, your Well-Being Plan offers steps you can take to reach your goals ? from little adjustments to big changes. You'll also receive a Well-Being Report that summarizes your current health and provides suggestions for improvement.
