SCOPE AND CONTENTS OF THE RECORDS

License applications and records, correspondence, Health Department inspection and compliance reports, and miscellaneous other studies and reports regarding licensure and inspection of hospitals, nursing homes, maternity homes, sanatoriums, treatment centers, group homes, and other health care facilities. The files cover facilities that closed, changed ownership, or for other reasons were removed from the licensed rolls or their prior files closed.

The dates on the file list are those during which the facility was licensed. Some materials in the files may pre-date or post-date the licensure dates.

Two boxes of closed facilities files were accidentally destroyed at the state records center in 1997.

In several overlapping sequences, each alphabetical by name of community, thereunder by name of facility.
