These questions are to help your teacher understand a little bit more about you, your interests, what you like about school, where you think you might need help in school and what your teacher can do to help you work better in school.

For most of the answers you only have to circle words, sentences, numbers or letters so read the instructions carefully. Some instructions will have explanations and examples as well.

A In this part we ask you questions about what you like to do for fun and what your interests are. This is also your chance to tell the teacher something special about YOU!
