PA Driver’s Licenses & Photo ID Cards

Please complete this form in order to let the Office of International Programs know of your intention to apply for a Pennsylvania Driver’s License or ID Card. With this information, a letter verifying your enrollment can be created. Return this completed form to the Office of International Programs, and allow a few business days for your letter to be created.

I am aware that I need one of the following things to apply: (Please which one you have.)

_____ Social Security (SS) card

_____ Rejection Letter from SS stating I am not eligible for a Social Security number/card.

I am also aware that I will have to provide two forms of proof of residency in PA to apply for either the driver’s license or the PA ID Card. Acceptable forms of residency proof are:

• CURRENT UTILITY BILLS you had mailed to you, in your name (water, gas, electric, cable, etc. - NOT cell phone bills)

• TAX RECORDS (not W2s - they do not have your US address)

• RENTAL LEASES (it is suggested you take your entire lease agreement)

• Usually the local DMV office will often accept a US BANK STATEMENT that has been mailed to you (not a print out from the internet!).

*** If you do not have a second proof with an exact matching address, a LETTER from our office can serve as one form of Proof of Residency (just write ‘Letter’ in number #2).

In addition your SSN card and proof documents, you will also need to take the following original documents to the DMV. Information about the required processes for driver’s license and photo ID cards can be found here:

• Passport: including visa and I-94 (your arrival record – print it online at i94 )

• Current and all previously held I-20 forms (the current I-20 must have been issued for 1 yr or more and have at least 1 year of validity left when applying at the DMV).

• College ID with photo and transcript (official recommended)

It is suggested you do this process at the PennDOT location on Rohrerstown Road
