Supervises: All Home Care Staff – to include Nursing staff, Environmental Health Staff, Public Health Staff, PAT Staff.

Purpose of Position

To plan, direct, administer, coordinate and monitor home care and public health programs; and perform public health nursing duties as defined by Clarke county ordinances, state and federal law for Clarke County.

Essential Duties and Responsibilities

The following duties are normal for this position. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.

▪ Organizes and directs agency operations and functions. Acts as County Health Officer with responsibility for communicable disease control and environmental health. Administers communicable disease programs. Directs human health hazard prevention, detection and clean-up activities. Directs communicable disease control measures. Initiates reportable communicable disease outbreaks and investigations. Directs and monitors prevention measures and symptom recognition education.

▪ Acts as agency liaison with Board of Health. Coordinates public health activities with other agencies. Assesses community health care needs through health statistical review. Participates in Board of Health and other meetings as needed. Participates in health and disease prevention/health issue groups. Board authorized signatory for grants

▪ Maintains clear lines of authority/communication. Recruits and selects agency personnel. Plans and conducts orientation and in-service training. Evaluates employee performance and writes evaluations. Assigns and directs work. Reviews statistical and narrative reports for completeness, accuracy and compliance with deadlines. Attends job related continuing education programs

▪ Prepares and monitors annual department operating budget. Prepares and monitors grant application preparation. Monitors department expenditures and revenues. Approves expenditures. Researches alternate funding sources and prepares grant applications. Reviews and monitors financial reporting procedures and documentation for compliance with county, state and federal requirements. Plan & operate within the annual budget. Oversees staff in preparing monthly billing to private and third-party payers and development of annual financial report.

▪ In times of a public health emergency, performs emergency response role, as needed.

▪ Oversees client Medicare/Medicaid administrative procedures. Supervises care and caseload. Conducts staff meetings and provides case consultation. Reviews staff reports. Supervises clinical records for compliance with accepted professional standards and practices. Performs direct nursing care as able and needed.

▪ Oversees immunization clinics. Reviews records and audits immunization cards. Administers immunizations as needed. Provides advice and education to clients regarding immunization/vaccine reaction symptoms.

▪ Refers clients/community members to other agencies, such as social services, AEA, physicians, dentists, maternal health, WIC etc. Consults with nutritionists and medical advisors as needed.

▪ Oversees staff who provides information and education to the public regarding communicable disease, immunizations, nutrition, public health issues and agency programs. Prepares and gives presentations and educational seminars.

▪ Maintains knowledge of current ordinances, laws, rules and regulations effecting public health program administration; personnel administration and supervision; budget development and administration principles; nursing practices and principles; and organizational theories, structures and inter-relationships. Participants in continuing professional development and training. Consults with Community Health Consultant regarding new programs, program evaluation and staffing.

▪ Submits testimony regarding rural public health care issues. May attend and participate in state and national health care forums.

▪ Evaluates programs for effectiveness and statutory and regulatory compliance.

▪ Participates in County Board and committee meetings. Conducts department meetings. Prepares agendas or agenda items. Participates in special agency programs and events.

▪ Maintains confidential records where appropriate.

▪ Prepares and/or oversees annual department report, health education presentations, health program news releases, newsletter editorial material, pamphlets, memos, et cetera.

Additional Tasks and Responsibilities

While the following tasks are necessary for the work of the unit, they are not an essential part of the purpose of this position and may also be performed by other unit members.

▪ Enters data into computer database as needed. Answers telephones and performs clerical tasks.

Minimum Training and Experience Required to Perform Essential Job Functions

▪ Iowa registered nurse license, or temporary license issues by the Iowa Board of Nursing and 3-5 years professional, registered nurse experience, preferably in public health, or any combination of education and experience that provides equivalent knowledge, skills, and abilities.

▪ Ability to decide the time, place and sequence of operations within an organizational framework, as well as the ability to oversee their execution. Ability to analyze and categorize data and information using established criteria, in order to determine consequences and to identify and select alternatives.

▪ Ability to manage and direct a group of workers, including the ability to provide counseling and mediation. Ability to persuade, convince and train others. Ability to advise and interpret regarding the application of policies, procedures and standards to specific situations.

▪ Ability to utilize consulting and advisory data and information such as budgets, payroll records, claims, client charts. Agencies reports, audit summaries, disease bulletins, community health assessments, grant applications and reports, schedules, Board of Health manual, policy manuals, personnel policies, health care resource manuals, drug reference materials and organizational analysis.

▪ Ability to communicate orally and in writing with clients, Public Health personnel, physicians and other medical care providers, clients’ families, County Sanitarian, Board of Health members, County Board of Supervisors, community groups, hospital/clinic personnel, Community Services personnel, students, school staff and the general public.

▪ Ability to calculate percentages, fractions, decimals, volumes, and ratios. Ability to interpret basic descriptive statistical reports.

Physical Requirements

▪ Ability to operate equipment and machinery requiring simple but continuous adjustments, such as; computer terminal, typewriter, calculator, and medical diagnostic instruments.

▪ Ability to coordinate eyes, hands, feet and limbs in performing slightly skilled movements needed to operate medical diagnostic instruments.

▪ Ability to exert mildly heavy physical effort in moderate to heavy work, typically involving some combination of climbing and balancing, stooping, kneeling, crouching, crawling, lifting, carrying, pushing, and pulling.

▪ Ability to recognize an identify similarities or differences between characteristics of colors, forms, sounds, odors, and textures associated with job-related objects, materials, and tasks.

Environmental Adaptability

▪ Ability to work under mildly unsafe and uncomfortable conditions where exposure to environmental factors such as temperature variations, odors, toxic agents, noise, vibrations, wetness, machinery, disease, animals and or dust can cause discomfort and where there is a risk of injury.
