Notes:

Please ensure the following prior sending an e-mail inquiring about the arrival of the Certificate Of Good Standing (COGS): Ensure that the concerned registration authority has sent the COGS to QCHP/MOPH. Ensure that the COGS has arrived MOPH premises by confirming its arrival with the mailing courier.

Once confirmed that the COGS has arrived to MOPH premises, an e-mail can be sent to the above mentioned e-mail addresses including the practitioner name, the scope of practice and the concerned registration authority.

It is required to submit an application for licensing on the electronic/registration system once confirmed by QCHP's concerned officers that the COGS has been uploaded on COGS system.

Any concern or issue in regards to the registration team (mentioned above) must be reported to:
