Job Description

Post: Community Operations Manager

Location: Somerton House (with travel throughout NI)

Reports to: Head of Hospice Specialist Community Services

Responsible to: Director of Care and Quality Governance

Job Summary:

Reporting to the Head of Adult Hospice Specialist Community Services, this role will have clinical operational accountability for the delivery of safe, effective and high quality adult community services within NIH including 8 specialist community nursing teams geographically dispersed across Northern Ireland, Hospice at Home nursing service provision, Day Hospice, Outpatient services and Community Administration Team.

The post-holder will provide clear leadership and oversee the co-ordination of all staff within their span of control. He/she will liaise closely with members of the clinical multidisciplinary team to improve channels of communication, allow maximum utilisation of resources and promote continuity and co-ordination of the services within the sphere of responsibility.

The post holder will need to demonstrate a person centred and governance focussed approach leading high performing multi-disciplinary teams, effective partnership and collaborative working internally and externally.

Key Results Areas:

Service Delivery

• Lead and develop excellent patient care through continuous review of evidence based clinical standards and ensure these are within an agreed framework incorporating policies, protocols and procedures.

• Create a strong patient focused team that challenges organisational and professional barriers and boundaries.

• Act on own initiative and discretion to ensure workforce and service resource issues are effectively managed, ensuring the workforce is fit for purpose and meets the needs of the service and patients. Identify priorities as necessary and in conjunction with the Head of Hospice Specialist Community Services adjust the skill mix as required.

• Ensure that clinical practice is evidence based and that clinical guidelines are implemented taking into account the scope of practice within the clinical areas.

• Responsible for the monitoring of clinical standards.

• Promote a culture of continuous improvement which focuses on the provision of high quality safe and effective care, allowing staff to maximise their potential and underpinned by honest, open communication and team working across disciplines.

• Ensure that Adult Community Services are safe and effective and that risks are identified, quantified and minimised.

• Ensure Clinical and Team Leaders are supported in their decision-making and are responsible for leading clinical practice.

• Ensure all staff has a clear understanding of the care and environmental standards they are expected to maintain and take prompt action to rectify poor performance.

• Ensure that all relevant key performance indicators are achieved.

• Promote a learning environment for staff ensuring that the education, training, development and support needs of staff are in place and robust.

• Ensure the delivery of excellent quality care, achieving a high level of patient / carer satisfaction, and implement actions in the light of patient / carer feedback.

• Report to the RQIA Registered Manager for Adult Community Services within NIH, providing them with an assurance of compliance with the Independent Health Care Regulations (NI) 2005, ensuring that all recommendations arising from RQIA inspections are acted upon and implemented in a timely manner.

• Ensure that performance is monitored in accordance with NIH polices and procedures and that corrective action is taken where necessary to address deficiencies.

Setting Direction

• Support the Head of Hospice Specialist Community Services in the review, development, implementation and delivery of Adult Community Services, ensuring integration with corporate and directorate strategies and service delivery priorities.

• Support the Head of Hospice Specialist Community Services in the implementation of strategic and operational plans for the delivery of Adult Community Services, in line with local and regional strategies.

• Assist the Head of Hospice Specialist Community Services with service development proposals and business cases.

• Contribute to the organisations corporate planning, policy and decision making processes as required and ensure the organisations objectives and decisions are effectively communicated.

Development and Innovation

• Alongside the Head of Hospice Specialist Community Services, lead teams in the continual review and improvement of their services.

• Actively promote and support new ways of working and models of service delivery to improve Adult Community Services for patients.

Quality / Governance

• Ensure that the needs of patients and their carers are at the core of the way the NIH delivers Adult Community Services.

• Ensure high standards of governance, including the assessment, qualification and management of risk.

• Ensure that Adult Community Services within the post holder’s area of responsibility comply with all professional regulatory and requisite standards.

• Work to foster and develop an environment where nurses are valued and are working within The Code: professional standards of practice and behaviour for nurses and midwives (NMC 2015).

• Ensure that audit is undertaken on a systematic basis, and ensure that critical / adverse incidents and complaints are investigated and action plans developed in a timely fashion.

• Ensure staff keep up to date with current developments regarding patient care.

• Ensure staff maintain confidentiality at all times, including the storage of records and the transfer of verbal and written information.

• Take personal responsibility for own professional growth and development.

• Maintain clinical competence and credibility.

• Participate and co-ordinate projects as identified in quality initiatives.

• Contribute to the NIH’s overall corporate governance processes.

Professional Management

• Liaise closely with staff as appropriate to ensure sound governance arrangements across the service area and the provision of professional support and advice.

• Ensure that processes are in place to ensure that all professional policies, procedures and legislative standards are issued, implemented and monitored.

• Respond to operational / managerial issues where the situation is out of the experience / knowledge of the individual.

Financial and Resource Management

• Support the Head of Hospice Specialist Community Services in identifying cost pressures and monitoring of delegated budgets, to ensure effective and efficient use of resources, assisting with variance analysis and completion of financial action plans as required.

Leadership

• Be visible, assertive and accessible figure.

• Provide clear leadership to all staff within Adult Community Services and ensure the NIH has a highly skilled, flexible and motivated workforce to provide high quality care.

• Be responsible for the implementation of all relevant policies, to ensure policy translates into practice.

• Actively work to support and empower staff to develop and utilise leadership skills, ensuring they are invested in appropriate levels of authority to undertake their role effectively in an accountable manner.

• Contribute to a culture of positive performance management through leadership, setting and maintaining high standards of practice whilst promoting a culture of effective team working, continuous improvement and innovation.

• Facilitate the team to work across professional and organisational boundaries.

• Act up in the absence of the Head of Hospice Specialist Community Services.

People Management and Development

• Lead by example in practising the highest standards of conduct on accordance with NIH policies.

• Identify opportunities and work in collaboration with the Education, Innovation and Research Department to develop programmes of work that will support the NIH’s commitment to multi-professional education and training.

• Assist in the maintenance of systems to identify the education and training needs of staff with are underpinned by service priorities and organisational objectives.

• Participate in the organisations Professional Development Review. Review individually on a regular basis the performance of community staff. Provide guidance on personal development requirements and advise on and initiate, where appropriate, further training.

• Ensure the NIH’s sickness and absence policy is implemented within Adult Community Services and establish effective mechanism for regularly monitoring levels of sickness and absence, taking corrective action where necessary.

• Maintain good staff relationships and morale amongst the staff.

• Delegate appropriate responsibility and authority to the level of staff within your control consistent with effective decision making whilst retaining responsibility and accountability for results.

• Participate as required in the selection and recruitment of staff in accordance with NIH policies.

• Take such action as may be necessary in disciplinary matters in accordance with NIH policies.

Grade: 8a (£39,540 - £47,448 per annum)

Essential Criteria

• A healthcare related degree or relevant healthcare professional qualification and worked at least 2 years at middle management (Band 7 or equivalent or above) in a clinical setting

• Successfully demonstrate people management, governance, leadership and organisational skills for a minimum of 2 years

• Have experience of working with a diverse range of internal and external stakeholders in a role which has contributed to successful outcomes for patient care

• Have a minimum of 1 years experience in a lead role delivering objectives which have led to a significant (defined as contributing directly to key Directorate objectives) improvement in service

• Have an ability to effectively manage a delegated budget to maximise utilisation of available resources

• Full valid driving licence and acc3ss to a car to undertake duties associated with the role
