JOB SUMMARY

The post holder has overall responsibility for the management of the paediatric Unit which incorporates the inpatient unit and the paediatric ambulatory care unit. The post holder will have direct line management responsibility, ensuring continuing professional and service development for the in patient nursing team members and ambulatory care unit, which includes a G grade ward manager/senior sister, a G grade HDU senior sister and a G Grade ambulatory senior sister. Support staff within this area will also come under your management. As a member of the senior nursing team the post holder will deputise for the service manager as and when required. The post holder will liaise closely with medical staff, the multi professional team and nursing colleagues to provide a holistic service responsive to the needs of children and their families. The post holder will deliver clinical care for 50% of their time and the other 50% will be devoted to professional, personal and service development. Developing and maintaining clinical governance frameworks and strategies are an integral part of this role.

KEY RESPONSIBILITIES

This position is a key role in the functioning of the Paediatric unit. The post holder will be the most senior clinically based paediatric nurse within the trust and is expected to have a wealth of knowledge and information to be able to function within a variety of clinical environments. As an expert nurse, the post holder will respond to all paediatric medical emergencies and trauma calls activated within the hospital and be a resource of information and knowledge for junior and senior staff.

As line manager for approximately 45 members of staff, the post holder will ensure that the paediatric unit functions to its full potential with the care of child and family as priority. All staff will receive continued support in their role and guidance in their career progression. The post holder will also have management responsibility for service development, staff sickness and absence, recruitment and retention, disciplinary procedures and budgetary management.

Clear outlines of the key responsibilities are identified below:

1. To ensure the service reflects the philosophy of children and family centred care, with consideration of local and national recommendations/individual/ethnic/cultural/social and religious needs.

2. To take forward and shape the future provision of the children’s service, ensuring a framework for audit and evaluation.

3. To act as an expert nurse resource and clinical expert within the paediatric department and throughout the Trust.

4. Empower others through professional development and role modelling, to make safe decisions.

5. To develop, promote and facilitate the implementation of the Whittington Hospital NHS Trust Nursing and Midwifery strategy.

6. To enable and support staff to develop and implement evidence based practice in child health, developing guidelines and policies with the multi-professional team.

7. To be actively involved with education, research and practice developments within the trust.

8. To ensure robust standards and frameworks are in place, in line with current recommendations, to ensure children receive high quality family centred care at all times.

9. Responsible for ensuring the efficient and effective use of resources and budget, staff and performance management, staff education and training in line with service needs and effective communication channels within and external to the department.

10. To provide clinical leadership for paediatric nursing staff and advice to paediatric in patient staff/ambulatory staff and the multi-professional team.

11. To be proactive in developing cohesive working relationships within the paediatric service, with the multi-professional team and community services to ensure appropriate care for children.

12. Ensure clinical governance frameworks are in place and facilitate clinical governance strategies throughout the service.

13. To develop professional leadership to facilitate the development of paediatric nursing within the Trust.

• Provide expert clinical advice and supervision

• Act as a resource to departments and wards that manage children within the Trust.

• Act as a resource to supervise the paediatric sisters in the management of their individual nursing teams

• Identify the nursing skills required to care for the paediatric patient group: develop training programmes for staff to acquire these skills; mechanisms to assess competence in these skills and provide ongoing professional supervision of staff.

• To develop evidenced based nursing standards, ensure compliance with these standards and provide a framework for changing practice.

• Work autonomously to deliver expert clinical care to children and young people and liaise closely with medical staff.

KEY WORKING RELATIONSHIPS:

All members of the paediatric nursing and medical teams, the wider multi-professional team, including social workers, child protection advisor, Liaison HV, Allied Health Professionals, play specialists and teachers and the women’s health team. Nursing and medical and management colleagues within the Whittington Trust, Human Resources department, IM&T, facilities, Middlesex University and Audit and Development department.

1. PROFESSIONAL RESPONSIBILITY

1. To work according to the NMC Code of Professional Conduct and to carry continuing responsibility for:

- The assessment of care and health education needs for children and their families.

- The implementation and evaluation of programmes of care for children provided within a team/primary nursing system.

- The implementation and development of clinical supervision.

2. To present and act at all times in a professional manner, and ensure that all staff working within the Directorate do likewise.

3. To maintain clinical competencies, to keep abreast of current trends and to take responsibility for own professional development.

4. To supervise clinical practices, to identify development needs of staff and to participate in continuous in-service training for all grades of staff. Ensure that staff keep up to date with clinical and professional development in nursing.

5. To work in conjunction with other lead nurses and the Service Manager to develop and implement nursing practices, promoting research and evidence based practice. Act as a catalyst for change in clinical practices within the Women and Children’s Directorate and the Trust.

6. Responsible for the staff within the paediatric Unit (Ifor Ward and paediatric ambulatory care unit) with regard to: sickness/absence, disciplinary, grievance, recruitment/retention, appraisals, personal development.

7. To maintain patient confidentiality.

8. To develop personally, professional and academically in line with agreed personal development plan.

9. Provide professional leadership and support for nurses caring for children on the general inpatient unit and ambulatory care.

10. Work closely with colleagues to develop evidenced-based practice within this area of children’s nursing, contributing to the Clinical Governance Agenda.

11. Understand and articulate the limits of one’s own professional competence.

2. MANAGERIAL RESPONSIBILITIES

1. Have direct line management responsibility for staff and services within designated areas, ensuring that professional standards/safe levels of care are maintained and to deploy staff as appropriate.

2. Ensure that systems are in place and followed to enable the accurate, comprehensive and clear documentation.

3. To initiate, develop and implement plans/strategies for improvement of paediatric services in conjunction with the multi-disciplinary teams.

4. To promote close collaboration and cohesive working relationships within the paediatric team, maintain effective communication channels and a harmonious working environment.

5. To advice the Service Manager and Divisional Manager on the appropriate level of skill mix and resources required for the designated areas.

6. To take delegated responsibility for the budget, ensuring effective utilisation of resources and be the authorised signatory for the paediatric unit budget. To provide periodic reports on expenditure and to address shortfalls.

7. To participate in the recruitment, selection and induction process for staff.

8. To monitor staff performance, implementing corrective action when required ie. Disciplinary procedures, sickness/absence management and grievance procedures. Undertake appraisal for staff within area of responsibility; agree staff and service objectives and development plans.

9. To identify and implement necessary changes to meet the needs of children and their families and the service.

10. Participate regularly in, develop and initiate audit, quality initiatives and projects as required to develop the overall children’s service.

11. Undertake and promote research projects within the paediatric unit to ensure evidenced based medicine is Implemented within the unit.

12. Ensure risk management strategies are implemented within area of responsibility and that staff understand and enact the incident reporting procedures. Be proactive in implementing action/learning plans to reduce potential risks or recurrence of adverse incidents.

13. Be proactive in minimising complaints within the department seeking to provide local resolution wherever possible. Undertake investigations for formal complaints and prepare response for Service Manager or Service Director. Provide feedback to staff on complaints and develop strategies to prevent recurrence.

14. To be involved in the interpretation and implementation of guidelines from NSF, CHI and NICE in accordance with clinical governance strategy.

15. Develop and implement systems to monitor and control expenditure and investigate any areas of overspend within the unit and take prompt corrective action.

16. Ensure that sickness and absence is managed to ensure staff welfare and

Resources are optimised at all times and complete computerised database to ensure efficient monitoring and recording of such data.

17. Ensure that Paediatric staff are aware of all Trust and national polices and procedures and investigate and manage any breaches of these in consultation with the Paediatric Service Manager.

18. To ensure the ambulatory service reflects the philosophy of paediatric ambulatory care for children and their families, with consideration of local and national recommendations/individual/ethic/cultural/social and religious needs.

19. To develop nurse led services and enhance the role of the paediatric nurse in the paediatric ambulatory care unit (PACU).

20. Develop policies and procedures for children and young people within the Trust to ensure the care delivered corresponds with government policies and national recommendations.

21. Develop clinical policies to ensure that children and young people come to no harm while in hospital.

22. Develop and present business plans for the development or reconfigurement of

services within the unit and generate income to enhance the unit development and to allow the purchase of equipment for the ward.

23. Supervise staff rostering, ensuring that adequate skill mix and staffing are allocated

at all times. You will be responsible for the management of the staff on the paediatric unit on a day to day basis.

2.10 Redeploy staff within the paediatric directorate to maintain adequate staff and

resources at all times.

2.11 Formulate and develop business plans and strategies for the development of the

paediatric unit.

2.12 As a paediatric expert you will represent children’s nursing within the

working parties of the hospital, ensuring that policies and guidelines take account

and acknowledge the needs of children and young people.

213 You will be responsible for the budget of the paediatric unit and the maintenance

and replacement of equipment and selection/ordering of supplies. You will be

authorising signatory for the unit expenditure and cost improvement initiatives.

3 CLINICAL RESPONSIBILITIES

1. Maintain professional accountability at all times, including the planning, delivery and evaluation of care within the framework of the named nurse and/or team nursing.

2. To be a clinical role model for staff by working clinically within the ward and ambulatory care unit.

3. To develop and implement nursing/ multi-professional evidence based/best practice guidelines and policies.

4. To establish a partnership approach to involve parents and families as much possible in the planning and delivery of care, including health education programmes. To implement the named nurse concept and collaborate with the relevant health professionals.

5. To work in close collaboration with the Consultant Paediatricians and their teams, and the wider multi-professional team to provide high quality care for children and support for families at all times.

6. To develop expert practice in a number of areas of general paediatrics to ensure patients receive the optimum care while in hospital.

7. Provide expert advice on the care of children to other specialists within the trust. Be influential in the care children receive and implement policies and guidelines to ensure continuity in the care this client group receives within the trust.

8. Be a creative, innovative leader in future developing the services for general paediatrics

9. To deliver high quality care and treatment to patients within the paediatric unit and advice other members of clinical and non clinical staff on issues relating to children and young people within the trust.

10. To be aware of own clinical competency and seek opportunities to develop clinical knowledge and expertise.

11. To undertake other clinical procedures in response to the changes and development in service provision and patient’s needs

12. Work alongside the psychiatric/CAMHS/Social work team to facilitate the management of children with mental health problems attending hospital.

13. Develop and promote collaborative working relationships and effective communication between all members of the multidisciplinary team in order to ensure a high standard of coordinated patient care.

14. Administer drugs that are included in the unit’s patient group directives. Devise new directions as needed to ensure an efficient service is delivered to this client group.

15. Attend discharge planning meetings and case conferences e.g. Child protection. Present complex and sensitive information to groups of professionals relating to the care of children and young people.

16. Communicate with parents and families on matters relating to sensitive issues, such as child protection and bereavement.

17. To provide extended role functions such as venepuncture and cannulation, plastering, suturing etc to assist in the management and advise of the care of children across the trust.

18. Administer IV medication including chemotherapy.

19. To order and interpret agreed diagnostic tests according to departmental protocols and within structured guidelines of the unit e.g. blood, urine, and skin/allergy testing.

20. As the senior clinical paediatric nurse within the trust you will carry the emergency bleep for paediatric medical emergencies and trauma. You will advise the medical team in all issues relating to the care of children and young people who are critically ill.

21. Maintain skills in advanced paediatric procedures i.e cannulation/venepuncture, suturing, Intraosseous cannulation.

22. Deliver expert clinical advice on the management of children with high dependency needs, including Non-invasive ventilation and invasive monitoring. Ensure staff are trained and competent in caring for this client group by providing supervision and education for staff

23. Develop and implement individual programmes of care for children in conjunction with the medical staff. Give professional and clinical advice on the care of this client group and ensure that the medical staffs are working within the clinical guidelines of the trust and paediatric unit.

4. EDUCATIONAL RESPONSIBILITIES

1. To support the paediatric clinical facilitator in providing an effective learning environment and staff development for students and trained staff.

2. To ensure an effective learning environment for staff, learners and parents act as facilitator in the supervision and teaching of staff.

3. Ensure that staff are educated in new skills and procedures. Appropriately develop the NMC “Scope of practice” to meet service needs.

4. To ensure that all new staff receive an orientation programme, in accordance with the Trust’s policy.

5. To support the clinical facilitator in working in partnership with local higher education institutions to establish and develop relevant education programmes for pre/post registration students.

6. Provide education and improve and develop systems to deliver information to parents and families.

7. Provide in-house training within the paediatric unit and across the trust for those new to the role of caring for the paediatric patient, including medical staff and senior nurses.

8. Participate in the induction programs for medical and nursing staff.

9. Work alongside and support new and less experienced junior doctors as they gain new knowledge within the paediatric unit.

10. Assist with the training of doctors in practical procedures, and monitor the standards of these procedures to ensure a level of good practice at all times.

11. You will lead in the education and training across the organisation on issues relating to paediatric nursing.

5. QUALITY AND AUDIT RESPONSIBILITIES

1. To actively co-ordinate and contribute to research and development programmes within the Directorate in order to promote good practice.

2. To establish protocols of care. Set and monitor measurable standards of care and be accountable for maintaining standards.

3. Develop the audit process for monitoring and reviewing the quality of nursing services provided.

4. Ensure staff are familiar with national, professional and local quality issues relevant to the delivery of services.

5. Implement quality initiatives that improve customer care and enhance the interface between staff, patients and visitors.

6. To identify training needs to support practice development within paediatric nursing.

7. To provide formal and informal teaching within the trust.

8. To collaborate with other professionals to initiate and promote research that will include the Trust’s research and development strategy.

9. To develop and initiate benchmarking initiatives related to paediatric nursing.

12. Leads the development and facilitates regular reviews of all protocols and guidelines specific to paediatrics.

13. Regularly use advanced IT databases and packages for the continuous monitoring of staff and patient information. Formulate weekly data of staff sickness, agency/bank nurse requirements.

14. Monitor the use of equipment within the paediatric unit and select and purchase new equipment by working in conjunction with the procurement department.
