Should you consider meeting with a Medical Genetics specialist to discuss your family history of cancer?

This form asks about cancer history in yourself and your close blood relatives, including your mother or father, your brother or sister, your aunts or uncles, or grandparents. Please do not include cervical cancer or skin cancer other than melanoma.

Have you or a close relative had:

← Cancer before age 50?

← More than one cancer in the same person? (Examples include cancer in both breasts, breast and ovarian cancer, or colon and uterine cancer)

← Several close blood relatives with the same type of cancer? (Examples include a mother, daughter, and sister with breast or ovarian cancer)

← Cancers that appear to run in the family over two or more generations?

← A very rare or unusual type of tumor? Examples include:

o Breast cancer in a man

o Medullary thyroid cancer

o Pheochromocytoma or other neuroendocrine tumor

o Retinoblastoma

o Adrenocortical carcinoma

← Breast, ovarian, or pancreatic cancer and Ashkenazi Jewish, Icelandic, or French Canadian ancestry?

← Any other cancer related issues in you or your family that you are worried may increase your risk of developing cancer?

If you answered yes to any of these questions, you may wish to consider meeting with a certified and licensed Medical Genetics professional to discuss your family history and cancer risks.
