I am completing my bachelor-applied social work. I am in my final four weeks. I have just completed my second practicum with the local hospital as a medical social work student. [identify the standard/s]32. Presenting situation: This may be a particularly difficult piece of work, an ethical dilemma, or a complex process that you have been involved in. Describe your role within the practice being presented.Core competence standard/s demonstrated My role is to ensure the patient has a safe discharge destination. I worked with the Multi disciplinary team with a complex case. An eighty year old man diagnosed with advanced prostate cancer. The family only speak Mandarin. The patient is currently with the palliative care team for comfort cares. The family were informed the patient is in the end of life stages. There are afew key issues.The ward he was on is for Acute medical illnesses. The family were given private level of care hospital rest home options for quite some time. The family declined everytime for a combination of reasons. The patinet was being brought to hospital everytime he had a fever which was nearly every week. For the same treatment of antibiotics and them discharged home. This case was ongoing for approximately three months. It was already an active case when I came on board. The famiy had stated they would be getting their lawyer invloved as they were unhappy with the treatment they had received. [identify the standard/s] 23. Social work assessment: Your reflections as a social worker on what you needed to do in this particular situation.Core competence standard/s demonstratedI was working along with the senior supervisor in this case. My role was to attend every meeting, take notes, write up the case note, make phone calls to the family to schedule meetings and book the interpreter. I had a clear picture of the situation. [identify the standard/s]54. Plan: What actions you undertook to ensure there was a plan of action. What was your role in setting up this plan of action?Core competence standard/s demonstratedMy role was to ensure the family had list of all the private hospitals in Counties Manukau with vacancies at that moment. It was my role to call the preferred private hospital rest homes daily to ensure the bed was still available just incase the family suddenly decided to move the patient to a rest home. Interventions: What you did; how you did it; why you did it?Core competence standard/s demonstratedThe case was active for three months now which by the charge nurses statement was far to long for an acute medical ward. I was asked to organize a family meeting. I booked the interpreter to help me in explaining to the family the meeting times and venue. I organized for the doctors the nurses the palliative care team, my senior supervisor, the interpreter, and the charge nurse to attend. The charge nurse organized for the hospital lawyer to be present also. At the meeting I was asked to take notes. [identify the standard/s]46. Outcome: What was the result of your actions? Consider how you directly or indirectly made a difference.Core competence standard/s demonstratedThe result of my outcome was I advocated for my own learning. I took on board the direction I was given. I did my best to ensure I was being accountable for my role toward the case. I ensured my clinical notes were up to date on file for the staff. I ensured the meetings were confirmed and the family was in formed of any changes. [identify the standard/s]67. What informed your practice, e.g. knowledge, past experiences, supervision, agency expectations, other colleagues and professionals?How did you manage ethical dilemmas?If you did not demonstrate all the core competencies what are your reflections on how you would do this in the future?Any other factors?Cor competence standard/s demonstrated I actively observed the actions of the Multi disciplinary team in order to get the just of this kind of situation. I do not think I could have done anything differently as I am a student learning from my senior social work colleague. My practice was informed because; I felt a real empathy for the family. I ensured I did my best to support and advocate for them to the best of my ability. As a social work student I am bound to the contract agreement between myself my tertiary provider and the hospital. I need to follow the correct procedures in this case. Even if that means going against my empathy I feel for the family. It has informed my practice by identifying I will come across many more complex cases that I will most likely always have that empathy. However I cannot act on it and go against the smallest restriction what ever it may be. I am mindful I will come across more cases, it is very important that I can identify this now early on in my career, so that I may grow and learn from it as my career progresses. I did not think to organize a cultural support advisor for the family. As it happens there is no mandarin social worker or cultural support on staff however I was only made aware of this when the lawyer questioned it. This is a learning I am taking away from this situation.
