JOB TITLE: WOUND OSTOMY CONTINENCE NURSE

DEPARTMENT: NURSING CLASSIFICATION: EXEMPT

SUPERVISOR: CLINICAL EDUCATOR/QUALITY SERVICES MANAGER

JOB SUMMARY:

Serves as a clinical resource to agency staff on issues pertaining to patients with wounds, pressure ulcers, ostomies, fistulas and incontinence. Provides direct patient care for evaluation and education purposes. Establishes and maintains practice standards and provides ongoing staff education.

GENERAL RESPONSIBILITIES *

1. Develops, facilitates and coordinates a cost effective ostomy wound care program.

2. Develops Protocols & Practice Standards for managing patients with wound ostomy and incontinence care needs.

3. Monitor patient outcomes on an ongoing basis.

4. Serves as a resource person for WOC patient care practices and makes home visits as needed.

5. Works with the Clinical Educator/Quality Services Manager on the development of education programs specific to Wound Ostomy Continence patient care.

6. Provides educational programs to agency staff and community groups.

7. Participates in Multi-disciplinary patient centered case conferences, as appropriate.

8. Maintains data and statistical reports as requested.

9 Participates in Research projects related to WOC issues.

10. Participates in Quality Improvement.

11. Assumes supervisory on-call responsibilities and acts as a resource person to nursing staff.

12. Uses a customer-focused approach when collaborating with Agency customers including co-workers, patients, physicians, and staff from community agencies and institutions.

13. Assumes responsibility to maintain and upgrade professional knowledge and skills through attendance and participation in continuing education and inservice education programs.

14. Demonstrates the knowledge and skill necessary to provide care appropriate to the age of the patients served on his/her assigned visit.

15. Supports the Corporate Compliance Program by adhering to the Policies, Procedures & Department Manuals of the organization. Employee will attend all mandatory inservices and annual training programs relating to Corporate Compliance. Assures that activities the employee is responsible for comply with any federal and state regulations. Reports and corrects any such conflicts which may arise.

16. Performs other duties as assigned. (Not essential)

QUALIFICATIONS:

1. License and current registration as a registered professional nurse in New York and Pennsylvania.

2. BSN and minimum of two years experience.

3. Completion of an accredited Wound Ostomy Continence ET Nurse Program and current Certification in Wound Ostomy Continence Nursing.

4. Excellent communication skills and the ability to work well with others.

5. Good organizational skills.

6. Good judgement and problem solving skills.
