Software and Digital Media Registration

Use this registration form to identify and describe a University of Washington research project’s software or digital media to the UW TechTransfer Digital Ventures. Please fill out this form only after discussing your project’s work with the Digital Ventures team. Digital Ventures may also advise you to submit a separate Invention Disclosure.

Purpose of Software and Digital Media Registration

By completing this form, you will describe the components of your work and help Digital Ventures determine an intellectual property management and distribution plan. This information helps Digital Ventures document and secure intellectual property rights as necessary and fulfills obligations to external sponsors of research. This information will also be used to register copyrights with the US Copyright Office if appropriate.

Submitting the Software and Digital Media Registration

All contributors must sign individual certifications in this form. The principal investigator is also asked to certify the overall form and should therefore sign last.

When you have discussed your project with a Digital Ventures team member and completed the form:

1. Return a hard copy of the completed and signed form to

2. E-mail the completed electronic copy of this form to your Digital Ventures team member.

3. E-mail or send a CD or diskette with the source code for the software or digital work that the project has created.

4. E-mail any other relevant documents to your Digital Ventures team member. Supporting documents may include project participation agreements, grant and funding contracts, publications about the software/digital work, and correspondence with affiliates. Electronic copies of these supporting documents are preferred, but if necessary hard copies are acceptable.

5. If the process identified here poses difficulties for the proposed contributors, contact the Digital Ventures for alternate means of submitting disclosure documents.
