2015-2016 DQC Assessment & Accountability Recertification Exercise

EVALUATION ON

UPON COMPLETION OF HOMEWORK EXERCISES Email dqc@ when you have completed all of your

homework exercises. You will receive by mail a DQC seal to add to your original

certificate. Your name will appear on the 2015-2016 Hall of Fame

website.

PLEASE NOTE Remember your Schoology username and password. Passwords

will not be managed by KSDE.

In Schoology, you have the ability to save your progress and return to it.

The recertification exercise is open resource, and it is encouraged that you look up the answers. Please complete all work independently.

A score of 90% for all questions in the exercise is required to become recertified.

You must complete the recertification exercise to the satisfaction of DQC program staff in order to renew your certification.

If you decide not to pursue recertification, please send an email to dqc@

DEADLINE All recertification requirements must be completed by Oct. 16, 2015.

If you fail to complete all of the requirements to recertify, you will forfeit your certification and will have to complete the A&A final exam and recertification process again to restore your certification status.
