IMPORTANT NOTICE TO TEACHERS: The Guidance Department no longer keeps recommendations on file. Please work directly with the student. We recommend online recommendations if the student is applying through the Common App or Universal App. Guidance will support you if you need help.

Student: This form should be presented to the teacher or person at least 3 weeks prior to your college deadline date.

I am applying to colleges and would appreciate it if you would write a letter of recommendation for me. You may address the letter “To Whom It May Concern” instead of to the individual college so that this letter may be used for each of the colleges I apply to. My intended career path is __________________________________.

If you would like to submit the recommendation online for the schools with that option, you may do that instead.

1. If you need TRANSCRIPTS sent, you must stop by guidance and submit a completed Purple “Transcript Request Form”

2. Complete Activity Sheet, Pink Recommendation Request Form & Parent/Guardian Brag Sheet

a. Go online to and click on “Activity Sheet.” TYPE your Activity Sheet in MS Word or MS Excel, save it and print copies for each person writing a recommendation. (These forms may be used later on for scholarship applications, so they must be typed.)

b. Get a copy of the Pink Recommendation Request Form from guidance. Complete a form for each recommendation writer making sure that you check whether you are attaching a form or it’s an online recommendation.

c. (Optional) Get a copy of the “Parent/Guardian Brag Sheet” from guidance and have a parent/guardian complete the form. Make copies for each recommendation writer. (This can be neatly handwritten.)

3. Provide your guidance counselor & each teacher recommendation writer with:

a. Polite verbal request to junior/senior teachers and your guidance counselor

b. Typed Activity Sheet

c. Completed Pink Recommendation Form

d. Completed Parent/Guardian Brag Sheet (Optional)

e. Attach any specific paper forms you need completed (if necessary)

f. For online recommendations, use your application program to request a recommendation and school report electronically and turn in the Purple Transcript Request Form to your Guidance Counselor.

OR

For paper recommendations to be mailed, give your teachers an addressed envelope with a postage stamp and turn in the Purple Transcript Request Form to your Guidance Counselor.

4. Send a handwritten Thank You Note to everyone who wrote a recommendation.
