Enter the specimen details into the system using Requisition Entry. Go to Epic button > Lab> Requisition Entry. A blank requisition form opens.In the Submitter field, completion match on the organization submitting the specimens. The system assigns a requisition number.In the Patient field, enter the patient's name. If the patient has been with us before, their demographics will fill in automatically.If the patient has never been seen at Riverside before, a pop-up will appear.If the patient is client-bill, enter the appropriate demographic information and choose New.If the patient is patient-bill, choose New & Reg to create the new patient and register them.Billing status automatically fills in based upon the submitter chosen. There are times when this needs to be changed, based upon patient circumstances. See a supervisor at your location to learn about these situations.If the testing is to be billed to the patient, click the Reg button on the requisition toolbar to register the patient.In the Code field of the Diagnoses table, enter the diagnosis code from the form, if appropriate.You can also search by the name of the diagnosis.There are multiple rows to allow you to enter all the diagnoses included on the form.Enter the authorizing provider.In the Procedure table, enter the test being performed in the Procedure field. For most tests, specimen type and source will fill in automatically. If it doesn’t fill this in yourself by selecting from the list.When the form is filled out, click Create Specimens (Alt+A) to create the specimen. The specimen ID appears in the bottom left of the form, the specimen labels print, and the collection information fields are plete the collection information, including date, time. If you know the collector and draw type, fill these in as well, but they are not mandatory, as we don’t always know this information for drop-offs.If you are working in a location other than the Kankakee campus, like Bourbonnais, Coal City, Frankfort, or Watseka, click Receive to receive the specimen(s) into your location. (This allows the specimens to be added to a packing list, and is therefore not necessary at the Pavillion.)You can scan the paper requisition into the system using the Media Manager button on the tool bar at any time before you accept the requisition.To save your work and close Requisition Entry, click the drop down arrow next to Accept & New (Alt+N) and select Accept (Alt+A).If you have several requisitions to enter from the same submitter that share similar information, you can set default values so you don't have to enter that information manually for every form.After you fill out the first requisition, click Set Defaults then click the fields you want to carry over. The selected fields turn green and bold. When you're done, click Set Defaults Complete, and then click Accept & New to save the current requisition and apply your defaults to the new requisition. Associate Tests With DiagnosesIf a patient has multiple diagnoses, you can specify which diagnoses are associated with which tests.In the procedure table, the Dx column will fill in automatically if you enter the diagnoses before you enter the tests. When it fills in automatically, all tests are associated with all diagnoses.If you want to change the associations, type in the number of the diagnosis. In the screenshot below, the first test (APTT) is associated with the diagnosis in row 1 (Abnormal coagulation profile), and the second test (Lipid Panel) is associated with the diagnosis in row 2 (Hyperlipidemia, unspecified).To associate a test with mulptiple diagnoses, type them both, separated by a comma – for example: 1,2.Continue with the requisition as normal.Answer a Collection QuestionSome tests have questions about the collection associated with them. This is indicated by a small red stop sign with a question mark in the Q column of the procedure table. This question must be answered before specimens can be created and labels can be printed.Click on the red stop sign.Answer the question. Since we may not have this type of collection information on specimens from other locations, there is an option of Unknown for questions about fasting.Click Accept.Continue with the requisition as normal.Order a Miscellaneous/Generic Lab TestThere are some sendout tests that we don’t perform very often. Instead of building every test that could possibly be ordered in Epic, we use the miscellaneous test for these unusual send-outs.In the procedure table, type “misc” or “generic” to find the Generic Lab Orderable (LAB30108).Click the question mark in the Q column of the procedure table.In the pop-up window that appears, type the GOKEY information in the Order Comments box. It is not necessary to answer the collection question for generic tests on requisitions.Click plete the requisition as usual.Document Information as a CommentTo communicate information, like where a provider would like results to be faxed, you can document it as a comment on the requisition.To put a comment on the all specimens in ordered in the requisition, enter it in the comments box in the lower right of the requisition.Note that there are two tabs to choose from:Lab Req Comm has a yellow background. This indicates that it is internal to the lab only and won’t go in the patient’s chart.Req Comments has a white background. Any comments entered here will be visible in the patient’s chart.To enter a comment on only one specific test, click in the empty box under the C column of the procedure table.In the pop-up window that appears, you again have the option of entering comments that go in the patient’s chart (white background) or are visible only to lab staff (yellow background).Click Accept when finished.After documenting your comment, complete the requisition as normal.Add Draw Charges to Specimens Collected Off-SiteWhen we collect specimens at off-site locations, like nursing homes or similar facilities, we enter the orders using Requisition Entry. Because these specimens differ from other drop-off specimens, we need a way to add in our draw charges. Enter the requisition information for the specimen(s) as usual.Once you have created the specimen(s), enter Off-site Venipuncture under Draw Type.NOTE: If you collect more than one specimen, you only need to enter the draw type once. We only drop one charge per plete the requisition as usual, receiving the specimen (if necessary at your location) and accepting the requisition.As you can see here, the draw charge drops when you receive the specimen into your location. ................
