| | |Management of America members with choices and career goals. I am proud of the many strong leaders in our chapter. My |

| | |goal is to empower our members where they want to grow, and maybe a little bit beyond their comfort zone. Is there a |

| | |community project we can own? Did you know there are chapters in our country that get grants to develop their community |

| | |projects? Where is Sacramento on this path? Work and family is time consuming, however, perhaps we can enable each other|

| | |to make the time for others who have no work or family. I see a future where our chapter members will succeed in making |

| | |a difference in those outside of their normal business hours. Don't stagnate - volunteer! I joined our chapter when I |

| | |was first hired as an RN case manager. I sought inspiration, education, support and growth and our chapter enabled me. |

| | |Over the years, I have been on the board in multiple positions, including secretary, president, executive director, and |

| | |currently, vice president. The talent of my predecessors inspire me to take the initiative and collaborate with our |

| | |board and members to take our chapter into 2019. Please consider my leadership as your chapter president in continuing |

| | |the growth of our chapter, and initiating a community project. It takes a village! |

|Dr. Mariann Cosby |Treasurer |Dr. Mariann F. Cosby is the owner of MFC Consulting in northern California operating since 1991. She provides legal |

| | |nurse consulting, life care planning, medical cost projections, bill analysis, case management, and expert witness |

| | |services. She has extensive experience in clinical, management, teaching, and consulting positions in emergency |

| | |nursing, pediatrics, geriatrics/public health, school nursing, and correctional healthcare. Dr. Cosby currently |

| | |consults for California Correctional Health Care Services and is an Assistant Professor for California State University,|

| | |Sacramento school nurse credential program. |

| | | |

| | |Dr. Cosby is an active member of many professional organizations including Case Management Society of America (CMSA) |

| | |since 2002. In 2006-2007 she served on the Sacramento CMSA Chapter Education Committee and assisted with the |

| | |development of the education programs. As 2018 comes to a close, her first term as Director will be completed. |

|Deborah Moroles |Board Member |Deborah Moroles has dedicated her career to transitions of care in the greater Sacramento-healthcare industry for over |

| | |25 years. She has served CMSA since 2012. Deborah's career mission is to help bridge better collaboration between all |

| | |providers of care. |

| | | |

| | |In her spare time, she volunteers to foster care systems and loves being with her family! |

|Ruby Wilson, RN |Board Member |Ms. Wilson has been in nursing for many years. She entered the nursing profession immediately after graduating from high|

| | |school. |

| | |Ms. Wilson, is a dedicated member of Sacramento Case Manager Society (CMSA). She has served as director for the |

| | |Sacramento CMSA for ten (10) years or longer. |

| | | |

| | |Ms. Wilson recently retired from Veteran Affairs(VA) as a Case manager after forty-one (41) years of dedicated service. |

| | |Ms. Wilson, has been active in the community serving as an Expert Witness for the Board of Registered Nursing. Active |

| | |member of International Honor Society Sigma Theta Tau. Serves as a resource for her church. Recently purchased and |

| | |presented her church with an updated first aid kit with all needed supplies in case of an emergency at the church which |

| | |also included blood pressure equipment. |

| | | |

| | |While employed she served as a mentor and preceptor for the VA. |

| | |Ms. Wilson now volunteers with the Elk Grove Police Department. |

|Victoria Florentine, RCP, CCM |Board Member |Victoria is a certified Case Manager (CCM) and Respiratory Care Practitioner (RCP). She is the owner of Select |

| | |Respiratory Services which provides transitional cardiopulmonary case management and post acute respiratory care. |

|Priscilla Chai, BSN, RN, PHN |Board Member |Priscilla Chai is a registered nurse and is currently the Director of Care Coordination and Disease Management for |

| | |California ACO.  She graduated Magna Cum Laude from California Baptist University where she also received the Excellence|

| | |in Nursing Academics Award. Priscilla leads with excellence and compassion, she is extremely bright and quick to learn, |

| | |and her unrelenting passion for improving care delivery continues to inspire the people who have the pleasure of working|

| | |with her.  |

|Paul Lenser, BA, CAPS |Board Member |Paul is a Certified Aging in Place Specialist (CAPS). The CAPS certification was developed jointly by the American |

| | |Association of Retired Persons (AARP) and the national Association of Home Builders (NAHB) to address the overwhelmingly|

| | |preference of Americans over the age of 45 to age in place, which means living in your home safely, independently and |

| | |comfortably, regardless of age or ability level. Paul earned a BA in Business Administration from San Jose State |

| | |University. His interests include his grandsons, volunteering – such as Walk to end Alzheimer’s, Gardening, Camping and |

| | |occasional round of golf, President of Chamber of Commerce, |

| | | |

|Paul Borja, MSN, MBA/HCM, RN-BC, CCM, |Board Member |Paul Borja has been industry of caring for patients since 2005. His experience ranges from patient care in Home Health, |

|CMAC, CDONA/LTC, PHN | |SNF, neurology/ortho/oncology units and acute rehab, as well as served in leadership roles in SNF, acute care nursing |

| | |units (neurology, ortho/spine/oncology) and Utilization Management/Discharge Planning departments. He holds Master of |

| | |Science in Nursing with a focus in Community Health and  a Master of Business Administration with a focus in Healthcare |

| | |Management. He is currently in his Doctor of Nursing Practice program at Grand Canyon University. He has four |

| | |certifications in Case Management such as RN-BC (ANCC), ACM and CMAC (ACMA) and CCM (CCMC). He has been in the Item |

| | |Development Committee of the Commission on Case Management Certification (CCMC) for a year now and has served as a |

| | |Subject-Matter Expert. He is currently an Assistant Manager for Case Management at Kaiser Permanente in Sacramento and |

| | |an Associate Program Director for Careway Health Institute. With a well-rounded experience in the continuum, he will be |

| | |a great asset in different facets of this specialty. |

|Arlene L. Gonzalves, RN, MSN |Board Member |Registered Nurse with over 10 years’ experience of acute care and ambulatory care. Specialized practice in vascular |

| | |surgery as a case manager, pre and post op care. Currently employed at Sacramento VA Medical Center Vascular Surgery |

| | |Case Manager 2009 – to present. Experience with Oncology, Neurology and Stroke patients as well. National Nursing |

| | |Education Initiative (NNEI) 2016– recipient of scholarship award as a VA registered nurses seeking to advance a nursing |

| | |degree Certificate of Appreciation 2013– in recognition and appreciation of my commitment to support the shared vision |

| | |on Northern California Health Care System Nursing Service, consistent leadership role within the Specialty Clinic Care |

| | |Practice Group, presented by the Associate Director Patient Care Services of the VA Northern California Health Care |
