|Job Purpose: |You will provide customers with a quality service of care and support customers to maintain an|

| |independent lifestyle. You will be responsible for assisting customers with medication and |

| |liaising with other professionals to support the customer’s health and wellbeing. You will |

| |also be participating in the schemes activity planner, providing a range of social activities |

| |to our customers. |

Key Responsibilities and Accountabilities

1. To ensure that customers Care and Support needs are identified through a comprehensive Care and Support Plan which is reviewed every quarter or as and when required

2. To ensure and individuals assessed needs are met through a person centred care and support plan whilst maximising independence and quality of service

3. To ensure privacy and dignity is maintained at all times whilst delivering a high standard of personal care and support

4. To effectively and appropriately network with other partners and stakeholders which may include health and social care professionals, advocates and other representatives identified by the customer

5. To encourage customers engagement and participation in activities on scheme and within the local community

6. To support customers to maintain the cleanliness of their home on a needs led basis

7. To participate with other staff members to provide a laundry service for the customers

8. To participate in maintaining the scheme physical environment with the housekeeping team.

9. Supporting customers to access the schemes restaurant service.

10. To adhere to Midland Hearts cash handling policy and procedure.

11. To carry out administration tasks as required and commensurate with job role and responsibilities

Health and Safety

12. To be responsible for the health, safety and welfare of yourself and others at work and to undertake the health and safety duties outlines in the Midland Heart Health and Safety Policy commensurate with this position.

13. To complete accident and incident forms as required.

14. To comply with Fire Safety requirements of scheme.

15. To comply with Health and Safety at work Act.

16. To report breaches of the above of perceived risks within the scheme to the senior team leader.

17. Deal with obvious risks immediately i.e. spillages, blocked fire exits etc.

To ensure risk assessments are compiled and reviewed as required:

18. To undertake all training as required, to attend training courses when places are confirmed.

19. Staff must ensure they respect the confidentiality of all customers and staff information which they will become aware of.

20. Staff must adhere to the Midland Heart Equal opportunity Policy.

21. Regular attendance at staff meetings are mandatory.

22. Staff are expected to work Bank Holidays and weekends as part of a seven day week rota.

23. Staff can expect regular supervision from their line manager. They should be available and prepared for supervision.

24. Occasionally staff maybe require to provide cover at other locations appropriate notice would be given and other locations would not be for the long term.

25. To undertake any other duties commensurate with the role.
