-287389-37213900Supported Employment ServicesQuick Reference GuideGeneral Service Definition: Supported employment means competitive integrated employment, including customized employment, or employment in an integrated work setting in which an individual with a most significant disability, including a youth with a most significant disability, is working on a short-term basis toward competitive integrated employment that is individualized, and customized, consistent with the unique strengths, abilities, interests, and informed choice of the individual, including with ongoing support services for individuals with the most significant disabilities –For whom competitive integrated employment has not historically occurred, or for whom competitive integrated employment has been interrupted or intermittent as a result of a significant disability; andWho, because of the nature and severity of their disabilities, need intensive supported employment services and extended services after the transition from support provided by the designated State unit, in order to perform this work.Purpose: This document is provided as a reference to update the recent changes made to Supported Employment Services. A comparison chart is provided for your reference and additional changes to the benchmarks are discussed below. The training curriculum must consist of both lecture and hands-on activity delivered face-to-face or virtually for a minimum duration of twenty (20) hours. Training must be completed prior to job placement and must include the following topics:resume writing; or virtual portfolioinstruction on interviewing skillssearching for and applying for jobs onlinefinding and completing job applicationsmanaging Employer initial contactshandling conflictsnavigating public transportation and a review of Customer employment benefits, if neededTraining must also include specific topics based on the Customer’s needs as identified through discussions with the Customer, the VR Counselor, and other support systems identified by the Customer on the referral form. Training lasting more than four (4) hours must include a non-billable 30-minute lunch break. No more than five (5) hours per day may be delivered for any method. The Pre-Placement Training Report and Pre-Placement Training Survey are required as deliverables for this benchmark.SE Youth Work Readiness Training:Work Readiness Training is an optional service for youth that may be delivered in conjunction with providing other Supported Employment Services. The Work Readiness Training Report and Work Readiness Training Survey are required as deliverables for this benchmark. Career Support Inventory: Formerly known as a Supported Employment Individual Career Plan, the Career Support Inventory provides detailed documentation of the Customer’s support structure (needed and presently in place), identified strengths and capacities, and available resources relevant to the desired employment outcome. The Career Support Inventory template and progress reports are required as the deliverables. Supported Employment Job Placement: The Supported Employment Placement benchmark is considered met when the Customer reaches employment with a single employer in an integrated setting for competitive compensation. Job placement date is considered the day the Customer begins competitive integrated paid employment, meeting their vocational goal. The job must match the stated employment outcome on the Supported Employment IPE, as agreed upon by the Customer and the VR Counselor. Two new deliverables are assigned to this benchmark: (1) A CIE Checklist verifying that the job meets the requirements of Competitive Integrated Employment and (2) a Job Analysis Form that assesses the employment “fit” for the Customer. Progress reports are also required as a deliverable for this benchmark.In the case management system, move the case to “In Employment” status on the placement date or as soon as possible thereafter.Supported Employment 30-Day Job Retention: This is a new benchmark added to the array of Supported Employment services. The Supported Employment 30-Day Job Retention benchmark is considered met when one of the following has occurred:The Customer reaches a minimum of thirty (30) days of continuous competitive integrated employment at one employer from the date of job placement. If already employed, the Customer has worked a minimum of thirty (30) days of continuous competitive integrated employment at one employer from the date of referral acceptance.The job must match the stated employment outcome on the Supported Employment IPE, as agreed upon by the Customer and the VR Counselor. Progress reports are required as a deliverable for this benchmark.Supported Employment Stabilization: The Supported Employment Stabilization benchmark is considered met after the 30-Day benchmark has been achieved and when the criteria for Stabilization have been met. The criteria for Stabilization are as follows:The Customer should satisfactorily perform his/her job duties in a setting for competitive integrated employment.At this stage in the Customer’s employment, he/she is secure in understanding the duties and responsibilities indicated in the job description.Natural supports have been developed and established.He/she requires minimal job coaching.Stabilization may begin when the Customer demonstrates acceptable job performance and there is a reasonable expectation that satisfactory job performance will be maintained with the kind and level of support services being provided. The Counselor, job coach, employer, and the Customer should agree that the Customer has reached Stabilization. Progress reports are required as a deliverable for this benchmark.Supported Employment Transition: The Supported Employment Transition benchmark is considered met when a Customer reaches sixty (60) days of continuous, stabilized employment with one employer after the Stabilization benchmark has been met. The criteria for Transition is the following:The Customer is satisfactorily performing his/her job duties in a competitive integrated setting at the weekly work-hour goal identified on the Supported Employment IPE.At this stage in the Customer’s employment, both the Customer and Supervisor agree that the Customer is acceptably performing all assigned job duties.Natural supports are actively utilized.Extended services have been engaged as identified on the Supported Employment plan.A new template is required as a deliverable – The Long Term Support Plan (LTSP). This plan describes the natural supports needed to maintain employment, extended services provider, and how the Customer’s long-term employment support needs will be met. Progress reports are also required as a deliverable for this benchmark.Supported Employment Successful Employment Outcome: The Successful Outcome benchmark is completed no sooner than 90 days of stabilized employment after “Transition” to extended service. The criteria for the Successful Outcome benchmark are the following:The Customer’s employment must be continuous with a single employer for a minimum of 180 days from the date of Job Placement.Must be in a competitive, integrated work setting.At this stage in the Customer’s employment, all VR services have been completed.Both the Customer and Supervisor agree that the Customer is acceptably performing all assigned job duties.Natural supports are actively being utilized and required accommodations are in place.Extended services are being utilized.Progress reports are required as a deliverable for this benchmark. ................
