The purpose of the education is to review key elements of quality care and the physician I-form.The entire program takes approximately 15 minutes to complete.? New Core Measure for 2013: VTE ProphylaxisKey Points for NursingGreater than 2 million Americans are diagnosed with VTE annually, more than 50% of those developed VTE while hospitalized or within 30 days of discharge, according to AHRQVTE is a preventable hospital complication that can lead to the following:added inpatient costsmortalityincreased LOSData abstraction from SLUHN patient medical records for compliance with the new core measure begins with January 1, 2013 dischargesVTE prophylaxis is also part of the Meaningful Use programVTE Prophylaxis I form is available, to be completed by practitioner on day of admission. The I-form includes a risk assessment for appropriate prophylaxis. Prophylaxis should begin on the day of or the day after admission. Prophylaxis can be pharmacological, mechanical (i.e. intermittent pneumatic compression devices), or both depending on the patient’s risk.Nursing’s role in VTE prophylaxisFollow prophylaxis ordersIdentify patients at risk; communicate to physician if prophylaxis not ordered on admissionDocumentation of IPC is a key component to meeting the measuresIntermittent Pneumatic Compression Devices (IPC)IPCs should be initiated on the day of or the day after admission to meet the measureEnsure IPCs are placed correctly on the patientDocumentation in Care Manager:Document that IPCs are on as ordered on the day of or the day after admissionDocument reason if IPCs removed, e.g. pt. refused, removed for OR….Document patient education on IPCsEven though the data abstraction does not begin until 01/01/13, the potential benefit to patients and impact on quality care cannot begin too soon.
