The primary objective of this form is to provide constructive feedback to individual team members regarding their team skills and level of participation. The results from these forms will be compiled and fed back to the rated individuals anonymously. Only the faculty will know the identity of specific raters. Only on an exception basis, may the results affect the project grade of an individual if there is a clear pattern of excellence or deficiency noted. Return this form to your team mentor at or before CDR/PDR.

Instructions: In the left hand column below, list your name first, followed by the names of your teammates. Rate yourself and your teammates on each of the attributes listed. With this form, we are seeking the input from peer team-members for their insight about the RELATIVE contributions of the other members of their team to this team’s overall effort. Use the following rating scale:
