Safe Surgery Checklist ROP

• A Safe Surgery Checklist (SSC)is an important tool in improving the safety of a surgical procedure

• The checklist can reduce the likelihood of complications following surgery and often improves surgical outcomes

Tips:

o In order for the checklist to be effective, all members of the Perioperative Team need to be present

o The checklist is used for every surgical procedure in the operating room

o The checklist may be initiated at each of the three phases by any member of the Perioperative Team: the surgeon, the anesthesiologist or the nurse.

What do I need to know?

• A standardized Safe Surgical Check list is utilized in all Health PEI facilities

• The Health PEI Surgical Safety Checklist includes three-phases:

1. Sign In: Before the induction of anesthesia

2. Time out: Before skin incision

3. Sign Out: Before the patient leaves the operating room

• By using a standardized audit tool, observational audits are conducted twice per year, by a designated perioperative nurse. Audit results are reviewed with hospital management to ensure 100% compliance.

Required Organizational Practices (ROPs) are identified by Accreditation Canada as evidence based-practices, which reduce harm to patients. They are mandatory practices matched to specific Accreditation Canada Standards that organizations must have in place. There are 33 ROPs identified within Health PEI for the September 2017 survey visit.
