New Hire/Newly Eligible Benefits Instructions Letter 2014/15 School Year

Dear New or Newly Benefits Eligible Kyrene Employee:

On behalf of the Employee Benefits Department, we'd like to welcome you to Kyrene School District. If you are a new hire for a position or combination of positions in which you work 30 or more hours per week, you are benefits eligible. If you recently attained more hours with Kyrene and now work 30 or more hours per week, you are newly benefits eligible and will need to complete the online enrollment process in iVisions within 31 days of starting your benefits eligible position.

Waiting Period for Benefits to Start: If you elect employee benefits within the 31 days, your benefits will begin the 1st of the month following 60 days of employment.

Requirement and Deadline to Enroll: Due to your benefits eligibility you are required to complete the online benefits enrollment in the iVisions self-service portal within the first 31 days in your benefits eligible position even if you choose to waive/decline all offered employee benefits. If you are choosing to waive/decline coverage, the online system is how you notify the district that you do not want employee benefits and it is how you set up beneficiaries for the district paid $50,000 life insurance policy.

Very Important Information about Automatic Enrollment: Failure to complete the iVisions online benefits enrollment process within the 31 day period may result in your automatic enrollment into an employee only medical plan of the plan's choosing. This may have a negative financial impact to you. Please review the full notice about automatic enrollment posted online on the Benefits Information webpage.

What will I need to turn in to the Benefits Department after I enroll online?

Dependent verification if enrolling spouse and /or children on your medical, dental or vision plans. Examples, marriage license copy, birth certificate copies, etc.

Is there a monetary medical allowance paid by Kyrene to use for my medical plan? Yes. Kyrene pays as part of your total compensation package an annual medical plan allowance that can be used to purchase a Kyrene medical plan. This allowance fully covers the premium cost of your employee only medical plan if you choose the Standard PPO option or Health Savings Account medical plan. For 2014/15 the medical plan allowance is $4,992. If a lower premium cost plan is available, the allowance provided by the District will be lower for employees enrolled in that plan.

If I don't elect a Kyrene medical plan, do I get the allowance back in my paycheck? No. A waiver of the medical plans is also a waiver of the medical allowance. The allowance is a medical plan allowance and is to be used specifically to purchase a medical plan with Kyrene. All other health plan elections an employee makes are considered voluntary and the employee pays the full cost of the premium for the elections. The medical allowance cannot be used to pay for these voluntary elections.

Do I have to submit my social security number for benefits enrollment? Yes. Disclosure of your Social Security number is permitted under Section 7 of the Federal Privacy Act of 1974. For health insurance and other employee benefit plans; our vendors indicate that SSN's are mandatory to identify individuals enrolling for coverage. Further, a new Mandatory Insurer Reporting law, Section 111 of Public Law 110-173, requires group health plan insurers to report SSN's in order for Medicare to coordinate payments with other insurance benefits. The SSN's for you and those of your family may be requested for these purposes. If you have questions about when or why a particular plan requires your social security number, please contact the vendor directly.
