INSTRUCTIONS

This booklet is divided into the following three sections:

Section I

These scholarships require the Portsmouth High School Local Scholarship Application which is available on the PHS website or in the Guidance Department. Complete one application leaving the "name of the scholarship line" blank. Make copies for each scholarship for which you are applying and then fill in the name of the scholarship. (Ask teachers ASAP if you need a recommendation.)

Submit completed packets with all necessary attachments to the Guidance Department secretary on or before the deadline. (One staple only or paperclip.) (Do not put in envelopes or folders.)

The guidance secretary will attach a transcript to all applications and forward them to the proper committees.

Section II

These scholarships require you to write a letter to the scholarship committee. Some may require additional information and or attachments. Staple these to your letter. Put your name and the name of the scholarship on each page. You are responsible for submitting or mailing the letters/attachments to the contacts provided. (Do not enclose in envelopes, if returning to Guidance.)

Section III

These scholarships require their own applications. (You may obtain an application either on our website or from the guidance secretary.) You are responsible for submitting/mailing the completed applications with all the necessary attachments. If a transcript is required, requests should be submitted at least five (5) school days prior to the due date.

Pay close attention to all deadlines as failure to meet the deadlines could result in being ineligible for the scholarship.

Scholarship amounts are based on the most current information at the time of publication. Actual amounts may vary.

Occasionally, a listed scholarship may not be awarded. The applications for Section I are available on our website ()

or in the Guidance Office. The guidance secretary will attach your transcript to all

ALLEGIANT MANAGEMENT CORPORATION RUTH A. STEARNS MEMORIAL Scholarship (Renewable for past recipients) Given in memory of Ruth A.Stearns to a PHS graduate continuing on to an accredited program

in the health care field. Applicants must also provide an essay describing their plans, the school they plan to attend, relevant information about financial need, and factors that may help

the committee with their decision.

NOTE: Financial need is not necessarily the determining factor. DEADLINE: April 1, 2010 ATTACHMENT: Essay

A.P.T. members and/or scholarship contributors.

GEORGE T. COGAN MEMORIAL Scholarship (Renewable for past recipients) Multiple scholarships awarded to male graduates of PHS or St. Thomas Aquinas High School.

Awards based on character, scholarship, leadership, and need. Applicants must have been

residents of Portsmouth, NH for 4 years prior to high school graduation. Applicants must file

the FAFSA and submit with the local scholarship application a copy of the page of the SAR that

indicates the EFC.

Past recipients re-applying will also need two recommendations, transcript from current college

and SAR.

DEADLINE: April 1, 2010 ATTACHMENTS: SAR, 2 recommendations (teacher, clergy, counselor, employer, etc.),

Confirmation of 4-year residency in Portsmouth, NH

HENRY AND PHYLLIS COITO Memorial Scholarship Awarded to a graduating PHS senior accepted to a four-year program. Applicant must submit

an essay (750 words max) answering the question, "If we gave you one do-over for your high

school years, what would it be?" as well as a graded essay from an English class. Please

submit 3 copies of each.

NOTE: Payable the beginning of college sophomore year. DEADLINE: April 1, 2010 ATTACHMENTS: Essay and graded English essay

CREEK ATHLETIC CLUB Scholarship Scholarship is awarded to a graduating senior continuing in a 2 year, 4 year, or vocational/

technical program who lives in the New Franklin Elementary School District and either attended

New Franklin Elementary School or St. Patrick's School. The recipient must have lettered in a

varsity sport. Preference will be given to students who reside in the City of Portsmouth's Creek

neighborhood.

DEADLINE: April 15, 2010 ATTACHMENTS: One recommendation from a Varsity Coach in the sport in which

the applicant lettered
