1. What are Transformation Schools? Transformation schools are identified through a competitive proposal process and have four core components: o Every student participates in the Choice model in its entirety (i.e., Montessori, STEAM etc.) o The choice model is based on national best practices o Open enrollment districtwide (with some seats open to out of district students) o No academic or parental entry requirements

2. What does "no academic entry requirement" mean? Students do not have to submit report cards, assessment scores or take a test in order to apply and be accepted Students will have to demonstrate they were promoted for the incoming grade at the time of registration (i.e. if applying for 6th grade, show final report card that student was promoted from 5th grade to 6th grade)

3. How do I enroll in a Transformation School? Students must submit an application for entry-level grades to participate in the lottery process conducted by the Office of Transformation & Innovation. Please see the website for the entry-level grades at each school. Applications for each school will be available via online and paper formats

4. How do I complete a paper application? Paper applications are available at all Transformation Schools and the Office of Transformation & Innovation (9400 North Central Expressway Suite 401) The applications can be returned at all Transformation Schools and the Office of Transformation & Innovation Paper applications are available in triplicate copy: o Yellow copy = parents o Pink copy = school (or whoever turns in the application if not parent) o White copy = OTI

5. How do I complete an online application? The online application link is available on the Office of Transformation & Innovation website: choice There is a separate link for the English and Spanish application

6. What is the application timeline? The application window is open from Friday, December 1st until Wednesday, January 31st

7. What is the Choice School Selection timeline? When do I find out if I get into a Transformation School?

8. How do I show proof of residency? Proof of Residency is due with the application to verify the student lives at the address provided on the application. Proof of Residency is waived for students in homeless or foster care situations

Non-Dallas ISD students/residents: Proof of Residency must be submitted for the application to be complete. Parents/guardians need to provide ONE of the following to the Office of Transformation & Innovation: utility bill (gas, water, or electric) OR mortgage or lease agreement in parent/guardian's name OR Affidavit for Proof of Residency (if residing in another person's residence).

Proof of Residency forms may be submitted in one of the following ways: o Dropped off to the Office of Transformation & Innovation (9400 North Central Expressway Suite 401) o Picture or scan copy emailed to choice@

Please do not send pictures of any documents via text message. Instead, please use the choice@ email address.

Current Dallas ISD students/residents: Current students do not have to submit additional paperwork. Dallas ISD will verify the address you have on file at your student's school matches the address listed on your Choice School application. The Choice School application will not be entered into the lottery if the address at your student's school does not match the address listed on your Choice School application

9. Can students living out of district apply? Ignite MS, City Lab HS and Hulcy STEAM MS allow for out of district students to apply. Students living out of district boundaries need to provide proof of residency to verify their address For Mata, IDEA, and Solar Prep, only students of Dallas ISD employees that live out of district may apply

10. Can students transfer into a Transformation School? Students cannot transfer into a Transformation School. Students must apply through the lottery or be placed on the wait list

11. Is transportation provided? Transportation is provided to Transformation Schools for students living within Dallas ISD boundaries. Bus stops are school stops, not neighborhood stops

12. Can students of Dallas ISD employees apply, even if they live out of district? Students of Dallas ISD employees may apply, whether or not they live in Dallas ISD boundaries. Contact the Office of Transformation & Innovation (OTI) for details

13. Can anyone apply for Pre-kindergarten at Mata? Students applying for PK at Mata must qualify under district guidelines. Mata does not have tuition-based PK seats o Dependent of active military o Qualify for free or reduced-price lunch o Homeless o Current or former foster care status o Limited English proficiency
