Reminder: The Ohio Department of Health requires all 12th graders receive the Meningococcal vaccine prior to the start of the 2017-2018 school year. Children will not be permitted to start 12th grade without proper proof of the immunization. It is advisable to make an appointment for your child to receive these vaccines to avoid exclusion. As soon as your child receives this vaccine, please provide proof of immunization from your physician to your school office so that we can update your child’s school health record.

Ohio Law (ORC 3313.671) requires all 12thgrade students receive two (2) doses of the Meningococcal vaccine (only one dose if given on or after the 16th birthday).

Please notify the Health Office if there is a medical or personal reason why your student cannot receive the Meningococcal vaccine so their health record can be updated or if your student has an upcoming appointment.

If you currently do not have access to a family physician, upcoming clinic opportunities are listed below. Notify the Health Office if your student has an appointment scheduled or if there are questions.
