IMMUNIZATION REQUIREMENTS

Dear Kindergarten and Grade 1 Parents, Date_________________________

Minimum Immunization Requirements for All Children Newly Enrolled in Kindergarten or Grade 1 and less than 7 Years of Age:

5 doses of diphtheria-tetanus-acellular pertussis (DtaP), Diphtheria-tetanus- pertussis (DTP), or pediatric diphtheria-tetanus vaccine (DT) or 4 doses are acceptable if the fourth dose was administered on or after the fourth birthday;

4 doses of either oral polio vaccine (OPV) or inactivated polio vaccine (IPV), in any combination or 3 doses of all OPV or all IPV are acceptable if the third dose was administered on or after the fourth birthday;

2 MMR vaccine

3 doses of hepatitis B vaccine

1 dose of varicella vaccine (Chickenpox) or Chickenpox disease date

According to our records your child is still lacking required immunization/s. Please send in your child's health record and physical exam as soon as possible. Students lacking immunizations will be sent an exclusion letter if immunizations have not been complete, within 20 school days.

For your convenience you may wish to have this information faxed to your child's school.

1. Written proof that the immunization/s listed above has been given by a physician or health department.

2. A written statement and a time schedule, approved by a physician or

local health department, for the completion of the remainder of the immunizations.

3. A written statement from the appropriate party(ies) indicating that the child has "...been exempted from complying with minimum immunization requirements for religious, medical".

2293 N. Main St., Crown Point, IN #755-3655 call for clinic hours.

Your child must be accompanied by a parent/guardian. Previous immunization records must be taken to show officials. Also, your child should not be ill with a cold or other illness.

3 doses of diphtheria-tetanus-acellular pertussis (DtaP), diphtheria-tetanus-pertussis (DTP), or tetanus-diphtheria (Td) vaccine, or pediatric diphtheria-tetanus (DT) vaccine;

3 doses of either oral polio vaccine (OPV) or all inactivated polio vaccine (IPV). If a combination of OPV and IPV is used, 4 doses are required.

1 dose of measles (rubeola) vaccine, on or after the first birthday;

1 dose of rubella (German measles) vaccine, on or after the first birthday;

1 dose of mumps vaccine, on or after the first birthday;

2 doses of measles-containing vaccine are required for children in grades six to twelve and students in ungraded classrooms who are 11 to 12 years of age or older by August 1, 1999.
