PRE-SCHOOL* Current Physical Examination completed on the Illinois form that includes all required immunizations. Physicals are accepted as long as it is completed within one year of the first day of school. Records are due no later than October 15th. Transfer students are UNABLE to start until a current physical is on file after the October 15th State of Illinois deadline. These students are allowed 30 days from the date of enrollment to prove compliance with the State of Illinois immunization requirements. Students without proof after the 30 days are subject to exclusion from school.* Dental examination is required for K, 2nd, and 6th grades.* Vision examination is required for any student entering an Illinois school for the first time in any grade. TRANSFER IN STUDENTS FROM ANOTHER ILLINOIS SCHOOL DISTRICT – ALL GRADES* Present the last required State of Illinois school physical examination that was completed (usually it is a preschool, kindergarten or 6th grade depending on age of child). Transferring in students are UNABLE to start until the most current physical is on file after the October 15th State of Illinois deadline. These students then are allowed 30 days from the date of enrollment to prove compliance with the State of Illinois immunization requirements. Students without proof after the 30 days are subject to exclusion from school.* Dental examination is required for students in K, 2nd, and 6th grades.* Vision examination is required for any student entering an Illinois school for the first time in any grade. ................
