Annual Report and Species Collection Report

Information Sheet

The enclosed annual report form and species collection report are required to be filed with the Texas Parks & Wildlife Department’s permitting office no later than 14 days following the anniversary date and/or renewal date of your permit.

The report forms must be complete, signed and dated. If you did not conduct any activities during the permit year, please complete the annual report form as requested and on the collection report state “No Activity Conducted.” You are required to file a report even if you did not conduct any authorized activities or are not renewing your permit.

The following is designed to assist you with completion of the report:

• Answer all questions on the annual report.

• The number caught and released refers to those specimens captured, temporarily held for identification, banding, radio-collaring, etc. and then released alive at capture site.

• The number collected (live take) refers to the number of specimens that you took from the wild (dispatched or kept alive) and retained for research and/or educational purposes.

• The number salvaged refers to those specimens found dead that were collected and retained for scientific and/or educational purposes.

• The number of incidental mortalities refers to the number of specimens that died as an indirect result of authorized permit activities.

• Disposition of specimens, refers to what was done with the specimens (i.e. voucher specimen, donated, retained alive for scientific research, etc.). You may attach a separate sheet of paper listing the disposition of specimens. If specimens were donated, please attach a list of individuals who received donated specimens.

• Please sign and date both the annual report and species collection table and mail to:
