SUMMARY INFORMATION RELATING TO THIS POSITION

Post: consultant physician with an interest in gastroenterology

Base: queen elizabeth university hospital/gatnavel GENERAL HOSPITAL

Applications are invited for this consultant post which will be based at Queen Elizabeth University Hospital and Gartnavel General Hospital. The successful candidate will join a team of 13 established consultants and provide care for gastroenterology inpatients and outpatients across the two sites. They will provide endoscopy services at both Gartnavel General Hospital and Queen Elizabeth University Hospital, take part in the out of hours emergency endoscopy service and speciality receiving delivered at the Queen Elizabeth University Hospital.

The post offers the successful candidate the opportunity to develop a sub-speciality interest, joining colleagues with interests in hepatology, inflammatory bowel disease, clinical nutrition and upper gastrointestinal disease. The speciality is supported by Clinical Nurse Specialists in Hepatology, IBD, Viral Hepatitis and upper and lower GI Nurse Endoscopists.

The department has a strong academic background and close links with the University of Glasgow.

The post comes at a time of major changes in the way clinical services are delivered in Greater Glasgow and Clyde, and will be integral to the planning and development of Gastroenterology services in the city in the coming years.

Applicants must have full GMC registration and a licence to practise. Those trained in the UK should have evidence of higher specialist training leading to CCT in General (Internal) Medicine and Gastroenterology or eligibility for specialist registration (CESR) or be within 6 months of confirmed entry from date of Interview. Non UK applicants must demonstrate equivalent training.

Queen Elizabeth University Hospital, NHS Greater Glasgow and Clyde

The Queen Elizabeth University Hospital (QEUH) is a new purpose built hospital that opened its doors in May 2015. The hospital has 1109 beds and will provide medical care to a population of around 700000 people in west and south Glasgow. The hospital provides the full range of medical and surgical services including many tertiary specialties including neurology, ENT and renal medicine. It will also be one of the 4 hospitals in Scotland to admit major trauma cases from next year.

We enjoy close links with Glasgow’s three universities and make a significant contribution to teaching at both undergraduate and postgraduate level. Research also has a high profile within the organisation. We provide excellent facilities for students and their tutors in the Wolfson Medical School, based at Glasgow University.

The Work of the Gastroenterology Department

The QEUH houses the department of Gastroenterology which provides all specialist in-patient and outpatient services to the population of South and West Glasgow. The new post will join the team of 13 current consultants to deliver this service.

A dedicated gastroenterology on-call rota has been set up to provide a 24 hour GI bleeding service. The on-call gastroenterologist will also cover 12 beds in the receiving complex where emergency admissions with gastrointestinal problems will be admitted.

The successful applicant will also contribute to the out-patient gastroenterology services both clinics and endoscopy. The current consultants have a range of interests including inflammatory bowel disease, hepatology, oesophageal disease and interventional endoscopy. Capsule endoscopy is available in the unit as is pH monitoring and manometry.

There is a purpose built endoscopy unit within the new hospital which will offer urgent emergency endoscopy to the in-patients as well as ERCP. There are two out-patient endoscopy units including a 5 roomed purpose built unit at the New Victoria Hospital and a 3 roomed unit at Gartnavel General Hospital. All endoscopic procedures including balloon enteroscopy are offered in these units.

This is a replacement post following retiral from the service. The successful applicant will be encouraged to develop and pursue a sub-specialist interest working alongside other colleagues in the department.

It is expected that 9 programmed activities per week will be devoted to direct clinical care, with 1 programmed activity allocated for supporting professional activities.There are extensive teaching commitments and the successful applicant will be encouraged to become involved with the teaching of undergraduates and postgraduate

c) Number and grades of junior and non-consultant grade medical staff:

Trainees of all grades from FY1 to CMT 2 will be attached to the unit. In addition around 4 or 5 specialist registrars training in Gastroenterology will be attached to the unit. There is one staff grade doctor (Peter Briggs) attached to the unit currently with an additional speciality doctor post at interview.

d) Non medical staff attached to the unit

Other members of the multi-disciplinary include

Nurse specialists – there are 3 IBD nurse specialists, 3 nurse specialists with an interest in liver disease and 2 nurse specialists who run the pH monitoring and manometry service. There are also a number of nurse endoscopists.

There is a well established dietetic service in the hospital

There is a nutrition nurse specialist that is attached to the nutrition team

e) Details of the Post

Receiving Work

The new appointment will take part in the gastroenterology on call rota in a 1:13 basis. The consultant will be on-call for Monday am to Friday lunchtime or from Friday lunchtime to Monday morning. During the week the main duty of the receiving consultant will be to cover the 12 bedded gastroenterology receiving bay where any patient with GI related problems will be admitted and assessed. Any general medical patient that is admitted to this bay will also be seen. There will be two formal ward rounds per day but the consultant will be available for advice throughout their shift. At the weekend the receiving consultant will perform an emergency endoscopy list in theatre, review any GI patients that are admitted and be responsible for 4 of the beds in the receiving bay as well as provide emergency reviews to the wards.

Non receiving In-Patient Work

The gastroenterology team covers three 28 bedded wards in the new hospital. All patients with GI problems are cohorted here but the team also sees any general medical patients admitted to these wards. This post will take an equal responsibility for covering the wards with 12 of the existing consultants.

Out-patient workload

The new post holder will offer three outpatient clinics per week. Sub specialist interests are actively sought and if these coincide with the sub specialist interests of the existing consultant group then team based working will be encouraged. The out-patient clinics will be based at Gartnavel General Hospital campus.

Endoscopy workload

The endoscopy workload will be divided into lists in the new hospital and will service the needs of the in-patient service including patients with GI bleeding and out-patient lists. There will be daily in-patient lists from Monday to Friday. The out-patient lists will be performed in the dedicated units at Gartnavel General Hospital or the New Victoria Infirmary. The new post will have 1 endoscopy list per week and this will include a share of the in-patient lists at the South Glasgow University Hospital.

f) Non clinical duties

Administration

There is 1 PA for administration duties associated with this post. SPA time will be allocated within the job plan to support the post holders personal appraisal and revalidation work

Teaching

Undergraduates attend throughout their training. This involves a commitment both to supervise students in clinics and bed-side teaching. There is postgraduate teaching for junior staff in the directorate and a programme of nurse education. The directorate holds regular lunch-time general medical educational meetings. The Gastroenterology Unit also has a weekly programme of departmental educational meetings, clinical governance meetings and Pathology CPC meetings.

Research

There are audit and research opportunities including the possibility of establishing research links with colleagues in the University of Glasgow.

The current job plan has 1 SPA but if the successful application wishes to participate in other SPA activity including teaching and research then it may be possible to negotiate additional time in the job plan with the Clinical Director

In addition, the appointee will spend 4 weeks a year performing gastroenterology receiving duties and during these weeks it expected other elective commitments are cancelled. The appointee is also expected to contribute to the rota for weekend.

This job plan is negotiable and will be agreed between the successful applicant and the Clinical Director.  NHS Greater Glasgow & Clyde initially allocates all full time consultants 10 PAs made up of 9 PAs in Direct Clinical Care (DCC) and one core Supporting Professional Activities (SPA) for CPD, audit, clinical governance, appraisal, revalidation, job planning, internal routine communication and management meetings.  The precise allocation of SPA time and associate objectives will be agreed with the successful applicant and will be reviewed at annual job planning.

h) Teaching, audit, and research:

The successful appointee will be able to play a full part in the teaching activities of the gastroenterology department including undergraduate and postgraduate teaching. There will be a commitment to ongoing clinical audit and opportunity for research.

i) Administration and management:

The post holder will undertake the administrative duties associated with the care of his/her patients and in the running of the department.

j) Qualifications:

Essential: CCT in Gastroenterology and General (Internal) Medicine.

Desirable: Experience in Acute Medicine
