ANNUAL REQUIREMENTS FOR RENEWAL

PEZA – REGISTERED SERVICE ENTERPRISES

A.1. Due on or before 05 January of each Year

• Annual franchise Fee (PhP6,000.00)

• Penalty of one percent (1%) starting on the 2nd week of January, compounded every month thereafter

A.2. Certification of “No Derogatory Record” issued by the PEZA Police

• Zone Level

• Home Office

B.1. Due on or before 01 May of each year

• Audited Financial Statement for the previous year

• Original (Annual Surety Bond of PhP100,000.00, in favor of PEZA (not applicable to PEZA-registered security agencies)

• List of PEZA clients and gross revenues derived from them

• List of current employees (include designations)

• Mayors Permit for the current Year

• SSS Certificate / DOLE Certificate / BIR Certificate

B.2 Additional reportorial requirements

(For Freight Forwarding)

• Civil Aeronautics Board Permit (renewal if expired) – Airfreight

• Philippine Shippers’ Bureau Certificate/Permit (renewal if expired) – Sea freight

• Philippine Ports Authority (PPA) Permit to Operate/

Clearance for the current year

(For Customs Brokerage)

• Customs Brokerage Licenses for the current year

• Copy of Common/Surety Bond with Bureau of Customs

• Philippine Ports Authority (PPA) Permit to Operate/

Clearance for the current year

• SEC Cert. Of Corp. filing / Information – 100 % Filipino

(For Security Agencies)

• Regular PC/PNP License to Operate (renewal if expired)

• Copy of Disposition Report submitted to PNP-SAGSD (latest)

• Firearms Licenses (renewal if expired)

• Security Guard licenses and badge (renewal if expired)

(For Trucking)

• Franchise from LTO/LTFRB (Decision)

• Common Carriers Bond

• List of drivers and helpers (include bio-data, 2x2 picture, NBI Clearance, Police Clearance, and Barangay Clearance

• Philippine Ports Authority (PPA) Permit to Operate/

Clearance for the Current year

Please be advised, in this regards, that in the event of failure to comply with the above requirements, the company shall be de-listed from the roster of PEZA accredited ecozone service enterprises in good standing which is circularized regularly to all PEZA – administered zones.
