This is a 4 digit identification number which was assigned by the CON office to hospitals opened prior to January 1 1997 This number will stay with the facility through name changes mergers etc for history tracking purposes Satellite hospitals were given the same number as the parent hospital If two hospitals merged and received approval from licensure to be considered one hospital i e both facilities are located on contiguous acreage then the 4 digit number of the main hospital has survived and the other 4 digit number has been retired Effective January 1 1997 all existing and newly licensed facilities

are tracked using the unique AHCA facility number assigned by the licensure office

This is a 6 or 8 digit unique AHCA facility number assigned by the licensure office
