The criminal history check must include:a name-based search of the National Sex Offender Registry, a search of the State criminal history repository in the State in which the program is operating and the State in which the individual resides at the time of application, andsubmission of fingerprints for a national criminal history background rmation to include when submitting a request for Minnesota and national criminal histories:National Sex Offender Registry information may be obtained at . To obtain State and national criminal histories, please submit a completed Serve America Act consent form signed by the applicant (see sample),a completed fingerprint card(see sample below),appropriate fees:State criminal history : $8.00 for charitable non-profit, $15.00 for all others,National criminal history: $15.75 for volunteers, $17.00 for employees.You may also request information from the Minnesota Predatory Offender Registry (MN-POR) by submitting a completed MN-POR consent form (see sample) signed by the applicant. If submitted with the criminal history check, there is no additional fee.Where to send your request Send the completed forms, fees, and a self addressed stamped envelope to Minnesota Bureau of Criminal Apprehension, Attn: MNJIS – CHA Unit1430 Maryland Avenue EastSt. Paul, MN 55106You may “pay as you go” or set up a prepaid account. If requesting the charitable non-profit rate, you must submit a copy of your 501 (c) 3 with your initial request. Per the Serve America Act, an individual is ineligible to serve in a position covered under the Serve America Act if the individual:refuses to consent to the criminal history check; makes a false statement in connection with the criminal history check; is registered, or is required to be registered , on a State sex offender registry or the National Sex Offender Registry; or has been convicted of murder, as described in section 1111 of title 18.
