PCA Project CoordinatorBrightwood/Western Massachusetts Senior Care Options ProgramOverviewCCA’s Senior Care Options (SCO) program provides comprehensive medical and supportive services to frail elders living in home and community-based settings. Many members of the SCO program choose to meet their support needs through the consumer-directed Personal Care Attendant (PCA) program. This program allows SCO members to choose and hire workers to assist with daily activities designed to promote their independence, health and functional status. Many SCO members in Western Massachusetts have expressed a need for additional assistance, such as help in identifying surrogates, with their responsibilities managing their PCA services. This full-time position will serve targeted members of the Brightwood SCO program located in Springfield, Massachusetts. ObjectivesThis staff position will work with one or more of the four Brightwood SCO primary care teams to bring information, resources and support to targeted SCO members who have expressed the need for support in managing their PCA services. The PCA Project Coordinator will assist them with the responsibilities of self-direction, including the responsibilities of being an employer. Based on their responsibilities within the Brightwood SCO program and with individual members needing support, the PCA Project Coordinator will work with Western Mass and CCA leadership to develop protocols, policies and procedures for the successful integration of the PCA program into the CCA SCO model of care.QualificationsBachelor Degree required, Master of Social Work or related degree preferred Spanish language proficiencyExperience working with vulnerable populations, especially individuals with disabilities and elders, and their familiesDemonstrated communication and exemplary interpersonal skillsValid driver’s license and reliable transportationUnderstanding of and advocacy for consumer-directed philosophy; knowledge of interdisciplinary team-based care preferred Reporting RelationshipsThis position will report to the Brightwood Site Manager and work in partnership with the primary care team staff, targeted individual SCO members, their families, and vendor agencies when appropriate.The position will work with the Brightwood SCO primary care teams in Springfield and will require regular meetings with members in their homes as well as participation in team meetings.Duties and ResponsibilitiesIn collaboration with the GSSC and skills trainer, provide ongoing individualized, targeted education and skill-building to SCO members who express the need for such assistance which may include:An ongoing education strategy to build consumer-as-employer skills of members, including recruitment, selection, training, and management of PCAsOngoing training and support to help members exercise and maintain their own decision-making authority and advocate for themselvesOngoing member support with regard to management and communication of expectations and responsibilities of the PCAs Provision of an orientation to the SCO program Continued support in reviewing the member’s Individualized Plan of Care with the member to clarify his/her expectations of the PCA role and responsibilitiesWhen appropriate, assistance in developing the member’s PCA Individual Preference Plan (IPP) that articulates member expectations of PCA and expresses member’s individual preferences for PCA education, skill building and training and for member’s individual preferences regarding PCA communication with care team Broker external partnerships to expand individual member supportsIdentify and be the main liaison with external surrogacy and care management providers, such as Community Enterprises, to integrate these resources into the SCO program for members who need them in order to effectively manage their PCA services Through the IPP process, identify individual member interest in education opportunities and skill building needs for their PCAs; communicate those needs to CCA care team and local education providers and work collaboratively to respond to these needsExplore member interest in and need for a PCA registry serving Brightwood SCO members; participate in discussions with local education providers and the State about the development of a registry; explore the utility of connecting to state resources such as Rewarding Work. Communication, Monitoring and DocumentationAlong with primary care team staff, monitor member health, safety and welfare, as articulated in the Individual Plan of Care (IPC), through frequent contact including regular phone contact, home visits, and team meetings Participate in primary care team meetings as appropriate Communicate with primary care team members about and document evidence of member challenges with self-direction; along with members of the primary care team work with member to address and resolve these difficulties. ................
