Instructions for Final Report Forms, LSTA grants

The Final Report consists of the following items. All items must be submitted or final payment cannot be made. Please use the report forms furnished in this packet.

1. Narrative Report: Please complete the form and answer the questions to document the activities during the grant period. Attach responses to the questions to the completed Narrative Report form.

2. Financial Report: List only LSTA Grant funds. Do not include local funds on the chart. Please use exact figures. Do not round.

LSTA Amount Approved—Enter the LSTA amount approved in each category as stipulated in the budget memo in the grant award packet. Do not change any of these figures. Changes can only be made after an amendment has been fully executed.

Amount Spent—List total amount of funds spent to date in each category, including the amount in LSTA Amount Spent This Period.

LSTA Unspent Balance—List what is left to spend. (LSTA Amount Approved minus LSTA Amount Spent)

3. Payment Summary: Expenditures should be arranged by budget category. List the items purchased, invoice number, invoice date, name of vendor, LSTA funds spent, local funds spent, and total amount of each invoice. For expenditures related to personnel include the name of staff as Vendor, number of hours worked and services performed in Description, hours worked in Quantity, position status in Invoice Number, time of service in Invoice Date, and wages paid by funding source. Please make as many copies of this form as needed to complete the list. Retain original invoices as documentation for this grant.

Example Answers to Final Report Questions

1. ACTIVITIES: Describe what the library did during the funding period (implemented, created, accomplished). Discuss how these activities contribute to meeting the needs of the target audience.

The library installed 4 self-check units, with the assistance of outside IT support. Two were installed in the Children’s Room and 2 by the circulation desk on the main floor. Over the course of the following week, the library fine-tuned all settings so that the units would function as desired.

Some troubleshooting was required. The units require patrons to enter a password, which was unanticipated. The library’s ILS allows for passwords to include both letters and numbers. The self-check-out software originally only allowed uppercase letters. The software company had not encountered this difficulty before and released a fix for this issue.

Staff members attended demonstrations of the self-check units in order to learn how to use the new machines, so they in turn could train patrons when needed. A link to additional training on YouTube was emailed to staff.

All activities contributed to the needs of the target audience by implementing a self-check system with features beyond what was previously available, and by improving the library’s technology infrastructure to better meet patron needs and expectations. New features included the ability for a patron to renew and check-out items without the use of a library card.

5. INTERACTIVE INSTRUCTIONAL PROGRAMS: If applicable, describe any interactive instructional programs offered during the reporting period. These programs include formal interaction and active user engagement. Responses must follow the example in the Resources section.

6. INSTRUCTIONAL PRESENTATION(S): If applicable, describe any low or no interactivity instructional presentations or performances offered during the reporting period. Responses must follow the example in the Resources section.

8. OUTCOMES: What results can be reported? If a survey was used, please summarize the results. Discuss the overall impact the project had on the library, the individuals served and the community. This includes changes in behavior, skills, knowledge, attitudes, etc.

EXAMPLE ONE:

The majority of patrons who completed the survey were pleased with the new laptop computers and photo-editing software. Several patrons remarked that they would not have access to the software if it was not offered at the public library. Four students used the software to complete a school project. A couple used the software to adjust family photos. Overall, participants left classes with an increased level of photo-editing skills and knowledge. All classes were full. The only complaint was that there were not more sessions held.

EXAMPLE TWO:

The digital scanner helps both staff and patrons to be more productive. The ability to email scans has appreciably reduced the hours that staff spend filling interlibrary loan requests through email. The ease of use has cut down on researchers’ time as well. Furthermore, the ability to scan fragile and oversize items has increased the availability of the collection to the community. The History and Genealogy Department purchased the scanner for their own use, but found that interest spread throughout the library. Patrons involved in scrapbooking and those preserving family memorabilia use the scanner with some frequency. Patrons who have not used a scanner before learn a new skill.

9. IMPORTANCE OF FINDINGS: Briefly summarize the importance of the outputs, outcomes, and/or other reported results. How will the findings impact your planning for future technology projects?

Providing technology and software to meet current patron needs and expectations helps the library stay relevant to the community. As a result of the favorable response we have had in the community to this project, we plan to be more proactive in our efforts to keep our technology up-to-date and add new technology when appropriate and funds are available.

12. PROJECT CONTINUANCE: Do you anticipate continuing this project after the funding period ends. Please describe.

The computers will continue to be used long beyond the grant period. The library will offer technology classes in winter of 2018. Library staff will maintain the computers and update the software on a regular basis to maintain reliability.

13. EFFORT LEVEL: Will the level of effort change following the end of the funding period?

The library’s effort to provide exceptional service to patrons will not change at the end of this grant period. We plan to develop and offer new classes for the public in Fall 2017. Topics will include Using Pinterest and HeritageQuest Basics.

14. PROJECT SCOPE: If the library will continue to use the equipment/technology and/or software after the funding period, will the scope of the project change? (i.e. providing training classes, adding additional technology for network infrastructure improvement, etc.)

Now that the early literacy stations have been installed, the focus of the project will shift to promoting the stations to children and their caregivers, tracking statistics, and maintaining the stations themselves.

Program Activities: We used our mobile lab to bring the Introduction to Facebook program to the local senior center. Participants learned how to create an account, edit the profile, home and account tabs, send and accept friend requests, review privacy settings and more. Participants were encouraged to set up their own Facebook accounts before leaving the session. A Facebook Basics Manual was provided for handy reference when using Facebook at home.
