Directions for “Student 1st Day Forms”Parent Activation Letter mailed homeOpen a browser on your computer and go to the following URL on the “Find it Fast” menu at the bottom left of screen and then click the “Student 1st Day Online Forms” iconLocate the Snapcode for your student, you will need this for the next step“The letter below is an example only, please use the information on your letter to log on.”Snapcode ScreenEnter the Snapcode located on your Parent Activation Letter for your studentClick “Continue”If you have previously completed “Student 1st Day Online Forms” you can click on “Continue Work”Account Creation/Sign in ScreenClick “Create Account” if you have never had InfoSnap/PowerSchool Registration account beforeIf you already have an account enter your email address, password and click “Sign In”Create an Account ScreenFill in all required fields and create your user accountYou will receive an email stating that you have successfully created an accountSnapcode Authentication PageThe name of the student which you are completing the forms forEnter the “Date of Birth” (MM/DD/YYYY) for the student listed in item 1Click “Continue”Introduction PageOnce you have created your account you will be directed to this page.Read the information and then click “Next” to continue*** If at any time during the process you need to pause you can click the account button and then click “Save & Sign Out” in the top right corner of any screen. Once you log back in on the Sign In screen you will be able to pick back up where you left off ***Student PageConfirm the information on the screen, make any corrections and provide answers to any area with a box. Once finished you can click “Next” to continueAt the bottom of the Student Page you will see an area that requires you to read the attached Emergency Closing Transportation form.You will click on the link “Emergency Closing Transportation form” and it will open so you can read the form. Once you have read the form just close the form that opened. Be sure Not To close the window that has the 1st Day Forms open on it.Select “Yes” from the pull down. Once you are finished click “Next” to continueParent/Guardian PageConfirm the information on the screen, make any corrections and provide answers to any area with a box. Once finished you can click “Next” to continueEmergency Contacts PageConfirm the information on this screen for person(s) that should be called in the event that a parent can’t be reached. DO NOT list the parent information again, this is only used if a parent cannot be reached first. Be sure to provide answers to any question with a box. Once you are finished click “Next” to continueHealth PageYou will need to enter updated health information yearly on your student. Be sure to provide answers to any question with a box.At the bottom of the Health Page you will see an area that requires you to read the attached Health Information form.You will click on the link “Health Information” and it will open so you can read the form. Once you have read the form just close the form that opened. Be sure Not To close the window that has the 1st Day Forms on it.Select “Yes” from the pull down. Once you are finished click “Next” to continueAgreements/Permissions PageThis page includes multiple forms that you will need to open, read and mark stating that you have read and or agreed to each form. Once you are finished click “Next” to continueElectronic Signature PageBoth you and your student will complete this screen stating that you both agree that all information provided on all screens is correct to the best of your knowledge. Once you are finished click “Next” to continueReview & Submit PageOn this page you can review all information you have provided and confirm that all required information has been provided.If the Submit button is greyed out, you have missed a required data field.The system will also tell you that you have not “Completed all required fields”Simply click on the “Edit” link beside the required box and the system will take you back to the screen so you can complete the page. You will mark the required field and click “Continue” to return to the Review & Submit PageOnce you have reviewed and completed all required information simply click on the “Submit” button to complete the process.You will receive an email stating that you have successfully submitted the Student 1st Day Forms for that studentSubmission Confirmation PageOn this page you have the option to print a copy of your submission for your recordsYou can also review any of the forms located on the websiteIf you have multiple students that you need to complete forms for you can click the link “Student 1st Day Forms” to begin the next student’s forms.If you are finished you can simply click on the account button and then click “Save & Sign Out” in the top right hand corner of the screenIf you have clicked the link “Student 1st Day Forms” located on the Submission Page you will see this screen.Simply type in the Snapcode for the next student and the process will begin again for that student.Accessing your account throughout the school yearAt any time during the school year you feel the need to look at any of the forms which you signed off on you can log back into the website using the account you created at the beginning of these directions.Once logged in you will see the “Student 1st Day Forms” for each student. Simply click on the link for your student and you will be directed back to the Submission Confirmation page where you can view any needed informationSubmission Confirmation PagePrint any needed information then click the account button and then click “Save & Sign Out” in the top right corner screen ................
