|Creative Horizon’s vision is to have a loving, nurturing and educated staff who partner with parents to help children develop the life skills and |

|Philosophy Statement |

|The educational program is based on sound principles of child development. Using the Montana’s Early Learning Guidelines and supported by Creative |

|Curriculum. Our goals are to give children a sense of self worth by helping them develop at their own individual pace while learning basic skills. We|

|want the children to feel confident and secure enough to explore and grow in an educationally stimulating and social environment. We are committed to|

|promoting and supporting all aspects of a child's growth in an atmosphere of respect. Your child is an individual who is an integral member of a |

|group, and whose uniqueness is acknowledged and appreciated by that group. |

|Creative Horizons shall strive to maintain our core belief that we and the world in which we live in began with and ends with the will of God. Our |

|teachings will reflect this principle. We will treat all children as if they are, children of God, with love, respect, tenderness, and dignity, |

|training them in a manner which is most approving by him. Creative Horizons shall strive to provide our children with as many of the essential tools |

|as possible, based on the abilities of each child, in order for them to not merely function, but excel in their adult lives. |

|Children participate in a variety of individual and group activities designed to develop language and reading readiness skills enhancing physical, |

|social, emotional and intellectual growth, skills and attitudes which will increase their ability to succeed in school. |

|The development and training of staff is a continuous process, which includes attending workshops, listening to guest speakers and frequent |

|discussions. College classes are encouraged. The staff is encouraged to take classes related to child development. The program has qualified teachers|

|and aides. |

|Smooth Beginnings |

|Welcome to Creative Horizons! To help you and your child make a smooth adjustment to the child care center, a classroom visit before your child |

|starts to meet the teacher is encouraged. The initial contact increases the child's anticipation and makes the child's first day go smoothly. We |

|would like you to know that sometimes it takes a couple of days for your child to feel comfortable, we encourage you to give your child a couple of |

|days to settle into the class. |

| |

|School Rules |

|Here at Creative Horizons, we strive to help the children understand the world around them, we have 3 simple rules that we use in our school: Care |

|for yourself, Care for others, and Care for your school. We encourage you to use these rules at home as well; Care for yourself, Care for others, and|

|Care for your home. |

|Daily Schedule ~ Our hours are 7:30am – 5:30pm Monday through Friday. In meeting our educational goals, we have designed a program that is both |

|consistent and flexible. Predictable routines help your child feel secure and increase their confidence. Schedules vary with each age group; they are|

|posted in each room and given to each parent upon request. |

|Curriculum |

|Creative Horizons uses Creative Curriculum by Teaching Strategies Inc., Mother Goose Time, as well as Montana Early Learning Guidelines as a basis |

|for all its lesson plans and activities. The daily activities are designed to provide appropriate experiences in the following early childhood |

|Each day includes both teacher-organized and child-initiated activities, indoor and outdoor activities, active and quiet periods and group and |

|individual play. |

|Meals |

|Creative Horizons participates in the USDA’S Child and Adult Food Program and receives federal cash assistance and shall strive to serve the highest |

|quality, nutritious meals.  Believing that nutritious meals play an important part in the development of the young child, we will only serve the |

|highest quality meats, fruits, vegetables, breads and dairy. Meals will be served in a family-style setting with proper table manners being |

|encouraged.  As part of our commitment to exposing our children to ethnic diversities, from time to time various ethnic meals from around the world |

|will be featured in the menu. We provide breakfast, lunch, and snack. Creative Horizons provides infant |

|formula and food to meet nutritional needs of infants. The food provided includes Similac Pro-Advance formula (blue), Similac Pro-Sensitive formula |

|(orange), baby food (fruits and vegetables), cereal, and snacks. If parents prefer a different brand of formula or baby foods offered, parents may |

|choose to bring their own. If treats are brought to the center for Birthday parties or other activities, they must be store bought and in a sealed |

|container or guaranteed to be nut free from home. |

|**No outside food is permitted in the center unless parents bring enough to share with their child’s entire class. |

|We are a NUT FREE Zone. Items containing ingredients such as: peanuts, peanut butter, almonds, cashews, walnuts, pecans, pine nuts, etc. are highly |

|discouraged! If your child eats any food containing nuts before they come to Creative Horizons, we ask that you wash your child's hands and face upon|

|entering the building. |

|Allergies and Allergic Reactions |

|If your child has an allergy or an allergic reaction to any food or drug, please make certain the child care center is kept informed and information |

|is always up to date. |

| |

|Breast Feeding Policy |

|Parents are welcome to come in and use the facility to breast feed their child. The Staff room is quiet and can accommodate privacy for the parent. |

|If the parent chooses, they may want to sit in the baby room and visit while they nurse. Breast milk is encouraged, and the center has the |

|accommodations for handling it appropriately. |

| |

|Assessment Policy |

|Creative Horizons uses Creative Curriculum for assessments on all children. Our observations are based on and were created to enforce Montana Early |

|Learning Guidelines and assessment levels through Creative Curriculum. These assessments are continuums which are completed twice a year. They also |

|consist of pictures which collaborate with the assessment. These assessments are used for teachers to create weekly lesson plans. The children’s |

|assessments are kept confidential between staff collaboration and parent. Parent Teacher conferences are done up to twice a year. |

| |

| |

|Special Learning Needs |

|If a child is assessed and shows a need for outside evaluation the parent will be directed to a specialist in the community. If a child has a need |

|for an IEP (Individual Evaluation Plan), IFP (Individual Family Plan) or any other specialized plan, the center can accommodate for this. Any outside|

|help will have to be initiated by the parent. The center can make provisions to be a part of any outside professional help for the child. Specific |

|daily schedules are posted in each room. |

|Guidance and Corrective Behavior We provide |

|an environment where corrective behavior occurs naturally through a combination of support, encouragement and limit setting. The following are |

|methods to encourage positive behavior in our centers |

|Diverting the child and redirecting him or her to a new activity |

|Having the child take time away from an activity |

|Providing a related consequence appropriate to the situation |

|If a developmental concern is noticed an in-depth evaluation is done to address concerns |

|If greater concerns the parent is directed to community resources. The center is willing to work with community resources to meet the needs of the |

|child. |

|Safe Environment and Ratio |

|Creative Horizons shall strive to provide a safe and secure environment.  Believing that the safety, security and well-being of our children is our |

|greatest responsibility, every precaution will be taken, and each staff member will be completely and thoroughly trained to maintain this |

|environment. Teacher to staff ratio is the following, 1:4 from ages six weeks to three years of age, 1:8 for ages three and four and 1:10 for |

|five-year olds. The front desk is monitored, and no child is to be picked up without the proper clearance. Weather and air conditions permitting, |

|outdoor activities will be planned for everyday. The classes will take the children outside twice a day for at least 20 minutes even during the |

|winter. Please make sure your child has warm clothes such as snow pants, boots, gloves and a hat. |

| |

|Release of Children |

|• The caregiver must not release a child to anyone other than a parent or legal guardian. |

|• Only persons designated by the parents or legal guardian on the Emergency Contact Information Form or by Parental Consent may pick up a child |

|(person must be at least 16 years of age and have a valid driver’s license). |

|• If the caregiver is not familiar with an adult, or does not know them, they must ask to see a photo ID, or they must not release the child until |

|the parent is reached by phone to check for authorization. |

|• If a non-custodial parent attempts to pick up a child, the caregiver cannot withhold their child unless there is legal paperwork in order. If the |

|non-custodial parent is not reasonable, the child should be released, and the police department should be called immediately with a thorough |

|description of the person and the vehicle. |

|• A caregiver should never put themselves, the child, or other children in any danger |

|** Closures may be added due to extreme weather or driving conditions. Notifications will be emailed and/or placed on the front door. We understand |

|this may be inconvenient, but we do not want a family or staff member to get in a severe accident trying to reach our facility in extreme conditions.|

|We want to take every precaution to ensure families and our staff are safe. |

|Mandatory Reporting |

|Creative Horizons and its employees are mandatory reporters. |

|Message Board & Community Board In the foyer is |

|a parent information board. Important schedule changes, forms requiring signatures for events and announcements are posted. Please check the board |

|regularly. We also have a community board that parents can add things to. |

|Health |

|The day a child starts and as age-appropriate thereafter, health records document the dates of services to show that the child is current with |

|immunizations according to the schedule recommended by licensing. Creative Horizons needs the parent to have on record the results of health |

|examinations (Statement of Health) for under two-year old’s, as well as up-to-date immunizations on all children for them to attend. |

|For your child to get the most out of his/her school day, he/she needs to be in good health. Do not bring your child if he/she cannot participate |

|fully in the indoor and outdoor programs. |

|Children should be kept at home when they are ill, have a temperature, or have other signs of illness. It is important that the persons listed on |

|your emergency card be available to come for your child in case of illness (with in an hour) or an accident. Please keep these numbers up to date. |

|The following are state rules and regulations that we must follow: |

|37.95.139 DAY CARE FACILITIES, HEALTH CARE REQUIREMENTS |

|(1) The parent(s) of each child admitted to the day care facility shall provide the name of the physician or health care facility the parent wishes |

|to have called in case of an emergency. |

|(2) If, while in care, a child becomes ill or is suspected of having a communicable disease reportable to the health department while in care, the |

|parent shall be notified by the provider. The parent is responsible for arranging to have the child taken home. |

|(3) The director, owner, manager, or person in charge of the day care facility must designate a staff member to check daily the health status of each|

|child immediately upon that child's entry into the day care facility, and to exclude any child showing symptoms of illness, under the following |

|guidelines: |

|(a) Children must be without fever of 101ºF or greater for 24 hours before they return to the day care facility, except that children with |

|immunization-related fevers need not be excluded if they are able to participate in the routine of the day care facility. |

|(b) Children must be without vomiting and diarrhea for 24 hours before they return to the day care facility. Vomiting includes 2 or more episodes in |

|the previous 24 hours. Diarrhea is defined as an increased number of stools, increased water in the stool, and/or decreased form to the stool that |

|cannot be contained by a diaper or clothing; |

|(c) Children with any of the bacterial infections listed below must be treated with antibiotics for 24 hours before they return to the day care |

|center: |

|(i) Strep throat; |

|(ii) Scarlet fever; |

|(iii) Impetigo; |

|(iv) Bacterial conjunctivitis (pinkeye); and |

|(v) Skin infections such as draining burn or infected wounds or hangnails; |

|(d) Generalized rashes, including those covering multiple parts of the body, must be evaluated by a health care provider to determine their cause |

|before the child can return to the day care facility; |

|(e) Children with chickenpox may not be admitted to the day care facility until their sores dry up, which usually takes 5 to 7 days. Day care |

|providers must not purposefully expose susceptible children to chickenpox, even with the permission of the susceptible child's parents; |

|(f) Children who are jaundiced must be excluded until a health care provider evaluates the cause and authorizes the child to return to the day care |

|facility; |

|(g) Children with symptoms of severe illness, such as uncontrolled coughing, breathing difficulty or wheezing, stiff neck, irritability, poor food or|

|fluid intake, or a seizure, must be evaluated by a health care provider before they may return to the day care facility; |

|(h) A child need not be excluded for a discharge from the nose which is not accompanied by a fever. |

|(4) If a child develops symptoms of illness while at the day care facility and after the parent or guardian has left, the day care facility must do |

|the following: |

|(a) Isolate the child immediately from other children in a room or area segregated for that purpose; |

|(b) Contact and inform the parent or guardian as soon as possible about the illness and request the parent or guardian to pick up the child; |

|(c) Report each case of suspected communicable disease the same day by telephone to the local health authority, or as soon as possible thereafter if |

|no contact can be made the same day. |

|(5) When a child is absent, the day care provider shall obtain the reasons, so the interest of the other children may be properly protected. If a |

|reportable communicable disease is suspected, the provider shall inform a health officer. No child shall be re-admitted after an absence until the |

|reason for the absence is known and there is assurance that the child's return will not harm that child or the other children. Disease charts that |

|identify the reportable diseases are available from the department. |

|(6) The day care facility may readmit a child excluded for illness whenever, in its discretion: |

|(a) The child either shows no symptoms of illness; |

|(b) The child has been free of fever, vomiting, or diarrhea for 24 hours; or |

|(c) The child has been on antibiotics for at least 24 hours for bacterial infections. |

|(7) The parent or guardian may also provide the day care facility with a signed certification of health from a licensed physician, except that the |

|following restrictions must be followed: |

|(a) If a child is excluded for shigellosis or salmonella, the child may not be readmitted until the child has no diarrhea or fever, the child's |

|parent or guardian produces documentation that 2 stools, taken at least 24 hours apart, are negative for shigellosis or salmonella, and the local |

|health authority has given written approval for the child to be readmitted to the day care facility; |

|(b) If a child is excluded for hepatitis A virus infection, the child shall remain excluded until either 1 week after onset of illness or jaundice, |

|if the symptoms are mild, or until immune globulin has been administered to appropriate children and staff in the day care facility as directed by |

|the local health authority. |

| |

|***Please note that fever free means without the aid of Tylenol and/or Ibuprofen. This only masks the symptoms, but a child is still contagious to |

|the other children, parents, and staff. |

|Medications |

|In order for us to give children medicine at school, state law requires that we have the following: |

|~Written permission from parent (State form) and from the child's doctor (prescription). ~Medicine labeled by a pharmacist with dosage and time of |

|administration. We need to send home all medicine every day. |

| |

|~We are unable to administer any over the counter medicine to any child under 2 years of age unless a doctor prescribes it in writing. |

|Parent Involvement / Participation We always |

|welcome your ideas, concerns, comments, suggestions and participation. Your participation in the school is important because it makes us partners in |

|the education of your child. We want to work with you to provide learning opportunities which match your child's own pace and style. It helps us to |

|know about family changes, such as a grandparent's visit or a new baby. We will also share our concerns with you. Feel welcome to spend time in your |

|child's classroom at any time and extended family is also invited. We look forward to getting to know each family. Since children are often with us |

|for several years, we see ourselves as a meaningful part of your children's lives. We hope that you will schedule talks with your child's teachers or|

|provider as well as take advantage of the formal conference periods held twice a year. You are your child's most influential teacher. Your |

|participation in the school says that you value education. Any concerns with the needs of the child or the staff can be resolved with the director |

|and a meeting can be requested at anytime. |

|There are a variety of ways you can help in the child care center such as: Sharing special talents and knowledge, sharing cultural heritage, |

|Volunteering in the class room, etc. The school holds a Mother’s Day event, Father’s Day event, Halloween Party, Thanksgivings dinner, a Christmas |

|program, and Valentine’s Party. All parents are encouraged to attend. |

|Things your child will need:(Label all your child’s items) |

|Infants~ bottles (2), sleep sack, extra clothes, diapers, wipes, pacifier (if needed), formula if different than regular or sensitive Similac, |

|swimsuit (summertime), snow wear (winter) & sunscreen |

|Toddlers~ light blanket, extra clothes, shoes, diapers & wipes, swimsuit (summertime), snow wear (winter) & sunscreen |

| |

|Two Year Olds~ light blanket, extra clothes, diapers & wipes (if needed), swimsuit (summertime), snow wear (winter), sunscreen & a water bottle. |

| |

|Preschool~ light blanket, extra clothes, swimsuit (summertime), snow wear (winter), sunscreen & a water bottle. |

|**Items such as toys are ONLY permitted on Friday’s for Show-N-Tell in our preschool rooms. No weapons or weapon like items. |

|Diapering and Potty-Training |

|Parents are required to supply all disposable diapers/cloth diapers and wipes for their child. If a child runs out and must borrow for more than 2 |

|days, a fee of $1.00 per diaper will be added to the parents billed account, this will not be refundable. (We cannot meet a diapering child’s needs |

|without the proper supplies.) Throughout the day, staff check children for signs that diapers or pull-ups are wet or contain feces at least every 2 |

|hours when children are awake as well as when children awaken from nap. Children are also changed when needed throughout the day if wet or soiled. |

|Staff and children are required to wash hands directly after each change or potty. |

|Clothing |

|Children are active in the center. They will be using expressive materials including paint (which may not wash out), water, mud, etc. In addition, |

|they will be involved in climbing and running activities. Please dress your child in comfortable, appropriate clothing. If you are concerned about |

|your child's clothing, please send a change of clothing. Please label the clothes--including all jackets, coats, etc.--to prevent lost items. |

| |

|Shoes |

|The children play very hard when at school, please bring them in appropriate shoes. Your child needs be to able to run and play safely so please make|

|sure that your child's shoes are appropriate. |

|Napping |

|The children take naps and they are no longer than 2 hours (usually 12pm-2pm). Each child has a separate mat, cot or a baby crib. The children’s |

|blankets are stored separately and washed each week. We follow all SID’s requirements and all staff that care for infants have SID’s training. We |

|also ask that parents refrain from dropping their child off during this time, unless necessary and a phone call is made in advance. It is very |

|disruptive for the class and wakes up many children that developmentally need their nap. When woke up early, it makes it hard on those children, the |

|teachers, and the child’s parent. Please help us ensure that children are not woke up early. |

| |

|Small Things |

|Check with your child care staff for his/her special requests on what to leave at home. Children should understand that anything brought to the |

|center should either be shared or left at home unless it is Show-n-Tell day. Candy, gum, toys, money, balloons and valuable jewelry should be left at|

|home. Children sometimes take small things home with them to remind them of school. If you find any small toys, puzzle pieces, etc., please return |

|them to your child's classroom. |

|Parent Concerns / Parent Rights |

|You’re Rights as a Parent in the Creative Horizons Program: As a parent concerned with your child's well-being and education, we encourage you to |

|bring your questions and concerns about our program first to the lead teacher whenever they occur. The first point of contact with questions and/or |

|concerns should almost always be with your child’s teacher. If you have a concern with the staff, please set up a meeting with the director or |

|assistant director. We have professional expectations with the staff, and they must follow the guidelines. You have the right to request a conference|

|with the director. There is variety of techniques used by the program to negotiate difficulties and differences that arise in interactions between |

|families and program staff. You also have the right to visit the center at anytime. We are trying to build a partnership with you and therefore we |

|encourage you to be in close and frequent communication with us. We have great respect for parents concerns or ideas and try at all times to address |

|these concerns and ideas. |

|Attendance and Absences Policies |

|Attendance |

|If your child is going to miss school for any reason, please call us as soon as possible at |

|All absences must be verified. Please call if your child will be absent or late. The parent is expected to state the reason for absence and the |

|provider is mandated by the state to ask. |

|Emergency Contact Form (yellow): It is important to keep your emergency contacts up to date. Teachers will release a child only to the parent or |

|someone authorized by the parent with proper identification, 16 years old or older. Please notify the front desk if someone not listed on the |

|emergency card is picking up your child. A request for exception to this policy must be in writing and must be approved by the director. |

|Confidentiality |

|Family records are confidential and kept in secured files accessible to authorized persons only. Written permission is required to release family |

|information. Confidential information is shared only with teachers, teacher aides, the Assistant Director, and the Director. Each room has |

|developmental portfolios on each child that has information such as assessments and evaluations that are done by the head teachers and shared with |

|the staff in the child’s room and passed on to the parents. |

|Out at the desk: |

|The child’s personal information files are seen only by the Director, Assistant Director, and Curriculum Director, each room has an emergency contact|

|list to reach each parent incase needed. |

|Field Trips: |

|ALL children in the class must be 4 for the class to participate in our walking fieldtrips. Children are never left unattended and the appropriate |

|number of staff members will always be present for the field trips. Staff will have cell phones on all field trips to call for help if needed. A |

|first aid kit is taken on each field trip for emergencies. A permission slip must be signed by a parent/guardian before a child can leave Creative |

|Horizons property to participate. |

|Coming and Going: |

|All children must be walked in and out of the classroom by parents or an adult designated by the parent. No child is to be in the parking lot without|

|an adult. Parents are required by state regulations to use daily sign in and sign out clock for documentation purposes. We also NEED you to sign your|

|child in and out for the food program; if they are not checked in, we do not get covered for that day. Children are not permitted to touch the sign |

|in/out clock at the front desk. A label is post at the top stating: Please Do NOT Let Children Touch This Computer. If a parent or child breaks the |

|Time Clock due to negligence, the full price for replacement will billed to that family and due immediately. |

|Fees: |

| |

|The rate for infant care is $850.00 per month, the rate for children 2 years old is $850.00 per month and 3 and older is $750.00 per month; We do NOT|

|provide part-time care. Payments are to be made the 1st of every month. If a payment plan is decided on your account, then payments are due on the |

|1st and the 15th of every month. If your payment has not been received, late charges will be added on to your account. If your bill is not paid in |

|full, your child will be unable to come back the 1st of the following month. *Rates are subject to change |

|A onetime nonrefundable Supply Fee of $35.00 will be charged to every family once per year. This fee is added usually in September, but it depends on|

|when a child(ren) first enroll in our center. |

|Late payment fees are as followed: A late payment fee of $25.00 will be added if you do not pay by 5 days after the expected date/dates. A return |

|check charge of $35.00 will be added if a check is returned from the bank for any reason. After 2 returned checks you will be asked to make other |

|arrangements for payments. |

|Family Vacations: |

|We understand that families plan time off during the year for vacations, etc. We, unfortunately, do not give credit to families for that time off. |

|Our center is licensed for 100 children and that would be a lot tuition not coming in during the year as we charge a monthly rate for a child’s |

|enrolled spot based on our center’s needs. If you have any questions, feel free to ask. |

|Late Pickup: |

|Hours of operation are 7:30AM to 5:30PM Monday through Friday with the occasional Holiday in which we are closed. The $2.00 per minute per child is |

|in effect. If no phone call has been received after 5 minutes (5:35PM), Creative Horizons will call the parents. If there is no answer, a message |

|will be left. After 10 minutes without a phone call, (5:40PM) we will call all Emergency Contacts and persons that are approved to pick up. If there |

|is still no answer from a responsible party, messages will be left. After 15 minutes without contact from anyone, (5:45PM) Child Protective Services |

|will be called and notified promptly. CPS or a Police Officer will pick up the child(ren) and the parent or a responsible party will have to retrieve|

|said child(ren) from their care. If a child is in Creative Horizons care after closing hours 3 times in 1 month, the child(ren) will be ineligible to|

|attend for a full week and no discounts on the monthly tuition will be made. Please call if you are going to be late or please make other |

|arrangements for your child(ren) to be picked up no later than 5:30PM. |

|Late Pickup Fee's: |

|Fees are charged as followed to both subsidized and non-subsidized families. Late fees are for services outside the contracted hours. Late Fee |

|Schedule for Subsidized and Non-Subsidized Families: $2 per minute per child and the late fee must be paid at the time of pick up. |

|Infractions: |

|One infraction is given after the third and subsequent late pick-ups. Child care services shall be terminated after three infractions for late |

|pick-up. Late pick-ups can be excused by the Child Care in unusual circumstances such as (severe storm, flood or closed freeway or bridge). |

|Termination of Care |

|Child care services shall be terminated for any of the following reasons: *Repeated failure of the child to |

|maintain appropriate behavior. |

|*Repeated failure to pay applicable fees on time. (see above policy) |

|*Repeated failure of parents/guardians to follow written policies. |

|*Repeated failure to pick child up at closing. (see above policy) |

|If child care services are terminated for any of the above reasons, the child will not be eligible for readmission for six (6) calendar months and |

|only if space is available. You may voluntarily withdraw your child/children from the center by notifying the supervisor. Ten days of absence without|

|notification will be considered a voluntary withdrawal. |

|Policy for Parents Terminating Care |

|If you would like to terminate care here at Creative Horizons, we expect a 2-week notice given to us in writing or verbally in order to fill your |

|child's spot. If you provide us with a 2-week notice, we will pro-rate the month for you. If we do NOT receive a 2-week notice, you will be charged |

|the full monthly fee. |

|Hours of Operation are 7:30am to 5:30pm. If late, pick up fees will apply. |

|Parents are asked not to drop off children during our nap time hours, unless necessary with a phone call, due to disruption. |

|Payments are due on the 1st and the 15th of each month. If not paid on time, late fees will be charged. |

|*If tuition is being supplemented with the Best Beginnings Scholarship, they may not cover full tuition and parents are |

|responsible for the transferred amount, no exceptions. |
